What is the Problem?
➔ Breast cancer is a prevailing issue that needs to be addressed. Cancer can be
silent so it is important to screen and do regular check ups as a preventative
measure.
➔ According to the CDC (2014), on average 1 in 8 women develop breast
cancer over the course of their lifetime in the United States.
➔ Statewide, epidemiologic data shows a rate of 114.8 per 1,000 women are
diagnosed with breast cancer, as shown by the CDC (2014).
What is the problem? (cont.)
➔ County data in Florida shows that Duval County has an age adjusted
incidence rate of 131.7 per 1,000 females for breast cancer (CDC, 2014).
◆ With this incidence rate, Duval County is the fourth county in Florida
that has the highest incidence rates.
➔ Healthy People 2020 Objectives
◆ “Reduce female breast cancer death rate”
◆ “Increase the proportion of women who receive a breast cancer
screening based on recent guidelines”
Goals and objectives
➔ GOAL: To decrease breast cancer mortality in Duval County.
➔ Process Objective: Over the course of our six month program, we will
conduct two bi-weekly meetings in zip codes 32254 and 32208 with 30 to 40
participants per meeting.
➔ Behavioral Objective: Six months after our program, 80% of participants will
have had a mammogram screening.
➔ Outcome Objective: Within five years, breast cancer mortality will decrease
by 5% in Duval County’s Health Zone I.
Target population
➔ Women living in Jacksonville, Florida

◆ Zip codes 32254 and 32208
◆ Aged 50-64
◆ Uninsured
◆ Low-income
◆ Primarily at or below 200% of federal poverty level

➔ Florida Breast and Cervical Cancer Early Detection Program
◆ Makes it easy to receive breast and cervical cancer screenings
◆ Screenings are free or at a low cost for women who meet the program
requirements.
➔ Baptist Health Needs Assessment
◆ Partners with the American Cancer Society, the National Football League, and
faith-based organizations to recruit women in Health Zone I who meet criteria
for receiving a free mammogram.
◆ Partners with the Susan G. Komen Foundation to provide free mammograms to
women who are uninsured or have not been screened in the past year.
◆ Provides community education on early detection of breast cancer and breast
health in Health Zone I.
