MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This agreement is made and entered into by and between:

The MANUEL V. GALLEGO FOUNDATION COLLEGES, INC. an

educational institution validly existing by virtue of the laws of the Republic of the
Philippines with office at Kapitan Pepe Subdivision, Zulueta Dist., Maharlika Hi-way,
Cabanatuan City, Nueva Ecija hereinafter referred to as the “SCHOOL” and herein
represented by MRS. LUZVIMINDA M. DE LEON, its Dean of the Nursing Program;

and

with office at________________________________________, Philippines, hereinafter
referred to as the “AGENCY”, herein represented by ___________________________
its _____(State Position)_____.

Witnesseth: That-

WHEREAS, the SCHOOL is recognized by TESDA to operate the

CAREGIVING program;

WHEREAS, THE AGENCY is an employment agency, duly registered and

accredited by POEA, that provides placement services for caregivers locally and/or
abroad;

WHEREAS, the SCHOOL wishes to enter into an agreement where the AGENCY
may consider placing the SCHOOL’s graduates for employment as caregivers;

NOW, THEREFORE, the parties hereby agree as follows subject to the terms and
conditions hereunder:

The SCHOOL agrees:

1. To submit to the _______________________________a certified list of

its graduates who have expressed a desire to avail of its placement
services;

2. To assist the students in complying with the documentary requirements for

application for placement and to coordinate with the
______________________ in scheduling the applicants for interview,
submission of documents and other requirements for placement and
employment.

II. The AGENCY agrees:

1. To identify its representative who shall be the one to coordinate with the
SCHOOL;

 2. To provide the graduates of the School with a copy of its application form;
requirements for employment; relevant policies and rules and regulations;
and any other matter to apply for employment on the relevant job
openings;

3. To place the SCHOOL’s graduates with reputable organizations and

employers who will provide them with reasonable compensation and
benefits and will treat them humanely.

4. To give the graduates of the SCHOOL a proper orientation on the culture,

expectations of employer, compensation and benefits, their rights and
obligations, matters to prepare for (whether, transportation, medical
concerns, emergencies, etc…) and other relevant factors that will affect
the graduates in their future work environment.

5. To give feedback to the SCHOOL of the areas of strength and weaknesses

of the graduates so that the SCHOOL can improve its training program.

7. To inform the school as to the status of application of its graduates; and

8. To update the school of the whereabouts of its graduates who are

employed by the agency locally and abroad.

III. The term of this contract shall be one (1) year for the S.Y. 2018 - 2019 starting
from the date hereof unless pre-terminated by either party by giving the other
party thirty (30) days written notice.

IV. This Agreement embodies the entire agreement between the parties. Any
modification must be in writing to be valid. Otherwise, it is null and void.
