2 CLASSIFICATION
Based on the location of the initial infection of the vertebral body, four forms of spondylitis are
known :
1. Peridiscal/paradiscal
Infection in adjacent areas of the discus (in the metaphise area below the anterior longitudinal
ligament / subcondral area). Many are found in adults.May cause compression, ischemia and
necrosis of the discus.Many are found in the lumbar region.
2. Central
3. Infection
Occurred in the central part of the vertebral body, isolated to be misconstrued as a tumor. Often
occurs in children. This condition often leads to early vertebral collapse compared with other types
resulting in greater spinal deformity. Spontaneous or traumatic compression may occur. It is found
in the thoracic region.
4. Interior
This pattern is thought to be caused by an aortic pulsation transmitted through a prevertebral
abscess beneath the anterior longitudinal ligament or due to local alterations of the vertebral blood
supply.
5. atypical shape
It is said to be atypical because it is too widespread and its primary focus can not be identified.
Included is spinal tuberculosis with only nerve arch involvement and granulomas occurring in the
spinal canal without bilateral involvement (tuberculoma), lesions in pedicles, laminae, transverse
and spinous processes, and lesions articular cells that reside in the posterior intervertebral joint.

The course of TB spondylitis disease there are five stages according to kumar, that is :
1. stage of implantation
Once the bacteria is in the bone, then if the patient's immune system decreases, the bacteria will
duplicate to form oloni that lasts for 6-8 weeks. This predilection generally occurs in the paradiscus
region and in children generally in the central region of vertebrates
2. early stages of destruction
After the implantation stage, subsequent vertebrate bodily destruction and mild narrowing of the
discus occur. This process lasts for 3 - 6 weeks.
3. stage of further destruction
At this stage there is still destruction, collapse of the caustic-formed vertebra and pus in the form
of cold abscesses, which occur 2-3 months after the initial destruction stage. Furthermore, it can
form sequestrum and intervertebral disc damage. At this time wedge shaped primarily in the front
(wedging anterior) due to damage to the vertebral body, which causes the occurrence of kyphosis.
4. stages of neurological disorders
Neurological disorders are not related to the severity of the kyphosis, but are primarily determined
by the abscess pressure to the spinal canal. This disorder is found in 10% of all complications of
tuberculosis spondylitis. Thoracic vertebrae have smaller spinal canines resulting in easier
neurological impairment in this area.
5. Stadium residual deformity
This stadium occurs approximately 3-5 years after the implantation stage. Kifosis is permanent
because of massive vertebral damage in front (Savant, 2007).
