The MOU is signed between Medicare trust hospital (Party 1) and Doctor Saima

Shakoor. FCPS, consultant in the subject/specialty of gynecology and obstetrics (Party 2).

By virtue of this MOU

Party 2 will undertake practice as a consultant in the prescribed specialty as per hospital
policies w.e.f 1st June 2009 for a period of one year.

The terms of references shall be as follows.

Practice shall be undertaken strictly under the disciplines, rules, laws, ethics of
constitution of Pakistan, and Pakistan medical and dental council.

All hospital policies shall be enforced during the contract the contract period and the
doctor shall abide by the rules.

Any modification or exemption, if requires by party 2 in the rules, in that case the matter
shall be discussed and entered into this MOU prior to implementation

Renewal of the contract can be made on the request of either party with in grace period of
one month with mutual consent of both parties and any new terms if any shall be made
part of this contract preferably at that time.

The MOU shall seize to hold if

Period of contract expiresa dn no request for renewal has been made within grace [period
which is one month at this time,

If any party desires to dis-continue the services by virtue of this contract however a
notice of one month shall be issued by either of the party otherwise the right of decision
shall be vested in party 1

Default from hospital policies by any of the party by malafide intentions

If party 2 repeatedly disobey rules policies of the hospital the contract shall be liable to be
cancelled or withdrawn with effect from the date fixed by the hospital however
immediate action may also be taken depending upon the gravity of the default, if being
grave

The rates/user charges have been detailed in this MOU as follows

Hospital patients are

1. Patients referred by the hospital GDMOs or administration
2. Patients of Zakat
OPD consultant fee

Consultant 40% of collection

Hospital 60% of collection

Operation charges

Major procedure from Rs 2000/= to RS. 4000 (i.e. caesarian section

Hysterectomy, myomectomy etc) sliding scale

Rs 2000 for Rs 8000
Rs 2500 for Rs 9000
Rs 3000 for Rs 10000
Rs 3500 for Rs 11000
Rs 4000 for Rs 12000

Medium procedure_________ Rs. 1000/=

Minor procedure ___________ Rs 1000/=

Private patients

• Patients brought by virtue of consultant own practice

OPD consultant fee

• Consultant 60% of the collection

• Hospital 40% of the collection

Ward admission i.e. patients admitted for monitoring or treatment purpose

without any surgical / diagnostic interventional procedure.

Consultant 60% of collection of OPD

Hospital 40% of the collection of OPD

Operation charges to be paid in hospital

Major procedure Rs. 8000/= (i.e. caesarian section,

Hysterectomy, myomectomy etc)
Medium procedure Rs 3000/= (D&C)
Rs 2500/= (Delivery)

Note: Hospital shall provide following facilities to the private patients.
