Engagement Performance Review Form

The Commonweath Contract Representative is required to undertake for each contract


an Engagement Performance Review. Evaluation of the Contractor’s performance is to be
measured against KPIs.

For further details please refer to Attachment K of the Deed of Standing Offer for Defence
Environment and Heritage Panel.

Instructions

1 The form required is dependent on the KPI of the Service Categories. Select
the form that is most relevant to your Service Category and generate form. In
some instances of overlap, you may be required to fill out both forms.

2 Once the Form is generated, fill in the details accordingly. All fields are
mandatory. If not applicable, please specify.
