AFFIDAVIT OF WITNESS

I, NEBULA MOON, Filipino, of legal age, married and a resident

of Mang Asar St., Brgy. Seven10, Iligan City. After having been duly
sworn to in accordance with the law, hereby depose and say:

1. That I am a public medical examiner for the City of Iligan;

2. That I work in the City Health Office as Senior Medical Examiner

III for eight (8) years;

3. That I personally know Mantis Earth who was of small built, 4 feet
and 11 inches tall, and of fair complexion;

4. That last January 20, 2017, at around 8:30 in the morning she and
her mother came to my clinic at City Health Office, 2F, Venus Avenue,
Iligan City for medical examination;

5. That Mantis Earth complained that she had been raped. And it took
them two (2) days before reporting the incident;

6. That I examined her body, I found injuries in her body part

particularly on minor abrasions in the elbows and contusions in the
legs and shoulders.

7. That on her reproductive part, I found that no lacerations in her

external organ or her hymen and no spermatozoa in her vagina;

8. That I issued a medical report from the City Health Officer entitled
Medical Certificate dated January 20, 2017;

9. That I am executing this affidavit to attest to the truth of the

foregoing statement that the complainant submitted herself to
medical examination to support the filling of the case against the
accused for violation of Article 266-A, par. 1 (b) of the Revised Penal
Code, as amended.

IN WITNESS WHEREOF, I hereby affix my signature this 4th

day of March in the City of Iligan, Lanao del Norte, Philippines.
