This home blood pressure record sheet instructs patients to take their blood pressure twice daily for at least 4 days, ideally 7 days, by taking 2 consecutive measurements at least 1 minute apart each time while seated. It provides a table to record the date, time, systolic and diastolic readings from the first and second measurements each time, as well as pulse.

We appreciate it is difficult but you should take blood pressure at least twice daily in the morning and evening. For each blood pressure recording, take two consecutive measurements at least 1 minute apart and whilst you are seated. Do this for a minimum of 4 days, ideally for 7 days.
