1 Mechanic & Electrician inform
Driller that he intends to
check the top drive.
2 Mechanic & Electrician
complete a permit to work
form and submit it to the
TP/Co. Man for review and
authorization.
3 Mechanic & Electrician climb
to the top drive with
maintenance tool using man-
rider winch.
4 Mechanic & Electrician
perform top drive check.
5 Mechanic & Electrician return
to rig floor.

Safe Condition or Activity Required
TP/Co.Man informs drill crew of operation to perform
maintenance check on top drive and risks associated
with the operation.
Before ascending attach maintenance tool to work
belt securely using rope or lanyard.
Mechanic & Electrician to use ride assist fall arrestor
and pay special attention that the maintenancetool
does not get trapped whilst ascending. Man-rider
operator to stand at man-rider winch console to
operate and safeguard.
Rest of rig crew except Driller to clear the floor area
during the operation to minimise exposure to falling
objects and alike.
Driller to stand by at console in case the drill line
(and thus crown sheaves) starts to move.
Mechanic & Electrician close out work permit with
TP/Safety Officer.
