adjoin land uses, access as well as existing facilities and utilities within 500 meters from the property boundaries of the project, drawn to scale and duly signed and sealed by a licensed geodetic engineer
4. Building specification and estimated

5. Zoning Certificate from HLURB Form available. Lawyer
Details to be filled up 6. Certified true copy of Environmental Clearance Certificate (ECC)
7. Certification from PHIVOLCS that
the property is not located within 5 easement from identified fault traces
8. Permit to drill from NRWB
9. Building permit
10. Two (2) copies of the Certified true
copy of title
11. Copy of the Current tax receipt of the
property
12. Right to use or deed of sale of right of
way for access road and other utilities if applicable 13. One (1) copy of project study/profile indicating the development cost, total project cost, amortization schedules, sources of financing, marketability, cash flow, architectural building plans and work program
14. Audited financial sheet for the last
three preceding years
15. Income tax return for the last three
preceding years
16. Certificate of registration from SEC Can get from
SEC 17. Articles of Incorporation Can get from SEC 18. By-laws Can get from SEC 19. TIN
20. List of names of duly licensed
professionals who signed the plans and other documents listed herein with the following details:
Surname, First Name, Middle Name,
Professional License No. (issuance and expiry date) and PTR with issuance date
