The following is only a small part of what I have had to deal with in the last 6 years.

Management is very aware of what goes


on at Jennings HBPC and nothing has been done. I have reported NUMEROUS times to management about what was going
on at the office, as well as my fear, stress and paranoia. Nothing will ever be done. It has taken a toll on my health. I have had
one doctor tell me, "find a new job" and another doctor tell me, "lose your conscience or retire".

These same employees have filed similar charges against John McBride, former HBPC Director, Harvey Norris, former HBPC
Director and others.

I have witnessed, listed but not limited to: falsification of records, false billing, missed meds and labs, I've been yelled at,
talked "down" to, had false charges filed against me, missed visits and billing, what I see as neglect of patients (supposed to
see patients every 30 days but patients not seen for as long as 130+ days), misuse of government vehicles, fraudulent time
clock use by others while I am held tightly accountable, etc.

Keep in mind I have used FMLA for 5 years. All of the health issues are the same except other issues have been added as my
health is declining. I should not have to apologize for trying to do the right thing for our veterans nor should I have to turn
my head and ignore what I see.

There is nothing I haven't seen and I guess my conscience is getting the best of me. Our veterans deserve better. Please take
note of the following information:

If we have an alarm on front door, maybe we need one for back door for security purposes. J 2/3 of employees, everyone
except Mike and I, go thru back door. If it's for security reasons, the back door is the perfect place for someone to come in
with an employee. It's in an alley.
Excerpts from my response to a hostile work environment I turned in, in 2012. My coworkers
filed hostile work environment against me but nothing came of it. I was moved immediately
from my office for 6 months but eventually was moved back.

I am writing in response to my letter I received regarding a hostile work environment. I have been in a hostile work
environment since the first day I came to work for HBPC but have tried to just let things work out on its own. I have expressed
my concerns to my supervisor on numerous occasions regarding the situation.

These are my findings as to why I believe they are filing hostile work environment although I feel I have been in a hostile work
environment since I started work there. I was too scared to say anything although others are aware of the situation.

Tabitha:

Tabitha was taking notes as to my comings and goings since I first started at HBPC. Every time I left the office, she
went to the front door/window to make sure I was gone (I actually saw her). She reported my every move to my
supervisor at the time, Rick Taylor.
2. In early December, 2011, Tabitha raised her voice to me because she felt I was double parked in the parking lot and
she had to park 3 cars down. This is a parking lot with no parking lines. She speaks very condescending and callous.
3. Tabitha requested an alarm be put on the front door so she could keep tabs on me but evidently did not want an alarm
on the back door as she is 10-20 minutes late for work EVERY day.
4. Tabitha yelled at me and also told me I was lieing in a conversation about the front door alarm.

I have a text message dated 8/24/12 to Kacie Nix, former HBPC nurse practitioner. I stated to her "I am afraid of her" —
referring to Tabitha. Kacie's response was, "Not physically but she's so sneaky you be careful bc she probably has
pictures & video of any little thing u have done or didn't do".

9. I have never reported Tabitha as I was afraid of her. I have put up with her yelling at me, mean attitude towards me,
her nail painting in meetings and behind closed doors, her religious emails being sent out, etc. I have looked over my
shoulder from the first day I came to work.
10. One of the nurses, Linda Jones, has stated to my supervisor that I was scared of Tabitha.
11. I have received condescending email(s) from Tabitha on more than one occasion (attached) and I have also emailed
Tabitha re: daily HBPC business but can't get any response (attached).
12. It has been called to my attention recently that I am not the only one who has problems with Tabitha. She has also
been reported to her supervisor by another dietician. She was also reported by Byron Singleton, HBPC Clergy.
13. I was having to email my supervisor every time I went somewhere or did something, as per my supervisor at the time
(Rick Taylor), because Tabitha was watching me and reporting my comings and goings.
I have spoken with Christine Mason, HBPC clerk, regarding the situation in Jennings on a daily basis and how bad it has been
(email attached). I have also spoken with Joan Fullilove, MH Social Worker, regarding how bad it is as I couldn't afford
counseling and she was lending a good ear.
This is not a normal email to receive from a coworker? Interesting for a coworker who watches
my every move.

I turn the chime off the front door when I am here.1 just got reamed again, by Tabitha, as usual, for turning it off. The
alarm/buzzer has been off since the day Rick came down for me to sign papers (over a month??) but she waits until no one is
here but me and her to say something. We are the only two here. I left and didn't turn it on when I left to go to lunch. I guess
she can't keep tabs on me when I do that. I don't like being spoken down to by an employee raising their voice, being called a
liar and I should not have to answer to her.

I explained to her it is annoying when I am sitting here with everyone going in and out. I told her when Rick came here he
stated it was annoying (hope you remember that Rick) and I made the comment I was going to turn it off while here. I was just
trying to get her out of my office. She then told me to "quit lieing". (the usual from her). She worries about the door in front
of me that she insisted we put a chime on but what is the difference, the back door doesn't have a chime on it? It seems
personal. Noone leaves out of the front door but me, and sometimes Mike.

Just giving heads up because I am sure she will file complaint, as usual. I guess it is my turn again. It was me, then Rhea, then
Kacie, now me again. Oh, I forgot, she had issues with Linda also. This has been ongoing since day one with me and others.

Heaven forbid I didn't turn that stupid chime back on OR double parked in a parking lot with no lines and she had to walk a bit
farther. She should worry about her own self coming in late every morning, the smell she puts out when she paints her
fingernails at the table during staff and inter-d meetings or in her office when it's just the regular employees here, the loud
religious tapes that we can hear playing from her office, repeatedly questioning a providers care of a patient in a progress note
to the point that it could get the V.A. sued and makes the provider look incompetent, the microwave she supposedly keeps in
her office because she is too good to use the one the rest of us peasants use or leaving early on days....and not worry about
me.

I am sick and tired of listening to her whining and attempts to belittle me. I am not the only one having problems with her and
at times, it has become serious. I can't continue to look over my shoulder every day. I have been having serious sleeping
problems; indigestion, nausea and vomiting at work and even had to leave work due to stress related issues. This situation is
crazy and I assure you, she will not stop.
The following is a text conversation I had with a former HBPC NP. I turned this in as part of the
hostile work environment investigation against me in 2012. I then sent it to the union in 2013 but
never received help or recognition of this hostile work environment.

Pictures were taken of my desk and turned in "anonymously" to the Privacy Officer. I have been
told it was the employee I am afraid of. Nothing become of the issue but as usual, I'm always
looking over my shoulder and management did nothing. I responded to Sandra Shirah, the
Privacy Officer and forwarded it to Ben Johnson, the "then" Union President. I received no

It is convenient how a day was picked that I was not here. It was announced in the inter-d meeting that I was working in
Alexandria.

The person who gave Ms. Shirah these pictures was in possession of illegally taken photos (against
policy) and I want something done. They are just as guilty as the one who took them. I feel
violated/intimidated and I am afraid and paranoid.

This is clear case of retaliation and I am requesting these illegal pictures being taken be pursued.
I've looked over my shoulder for 5 years I'm tired.

1. This "anonymous" complaint happens to come during an investigation of allegations of serious wrong doing in this
office by other employees interesting no one had a problem with me untif things were evidently brought to light
As stated in a previous email to someone in management last week," I am feeling the
pressure ........................I am sure the worst is yet to come." Evidently, the worst is coming. This appears to be
RETALIATION, which I was expecting. Why would I set myself up for this complaint when I know they are
watching me, as they have for the past 5 years?
2. Picture #1 shows a calendar date of February 8 but it supposedly took place February 17, a day I happened to have
worked in Alexandria. I returned around 4:30 and worked until 6:45 pm on the 17th. I have a witness who sits in the
office behind me, Cecelia Huggins, ext 4754, who I believe can vouch for my desk top. She is always going thru my
office to the copier. My calendar is put in my desk because it has information in it that I feel is noone's concern (my
leave balances, etc.). I happened to be trying to locate my paperwork around 4:45-5:00 because it was not just out on
my desk and visible in plain sight. I remember this date very distinctly because Dr. Loeb knocked on the door at about
5:00 and appeared upset. He had evidently became lost when trying to find a patient's house and that is why he came
back to the office late that day. He had been out seeing a patient.

3. Please make note that appears pictures #3, 4, 5 and 6 have been cropped. If they were on my desk, one would think
they would take a picture showing it on my desk, not close up. They took the pictures to make one believe they were
all visible but they would have had to have been spread out for it all to show ......... .....picture #2 shows that not to be
true.

4. Picture #2 is a picture of my desk with no identifying information on it. This picture proves I do not leave PHI
information unsecured on my desk in my absence. (Those scissors were just ordered 2/9/17 so this was taken after I
received them, which usually takes 3-5 days). There is a box under my desk that I put there on a day for safety reasons
until I could address it.

5. After reviewing the attached pictures (#3, 4, 5 and 6) I see NO identifying information to show that this is actually my
desk. The background is black????? As you can see from picture #2, nothing on my desk is black, nor is there anything
in my office that has that background. These pictures appear to be manipulated in some way. Seems to me an
employee is digging where they shouldn't and invading my privacy and making a hostile work environment!

6. Picture #5 of the email with my name on it has no PHI or patient information on it and could have been taken from the
printer and the picture taken at any location.

7. I have reported to Mr. McBride, I would say approximately a year ago (?), my concern about faxes being left on the
printer while I am on leave. I came back one time and there was well over 100 of them. To this day, not one
employee seems to have an issue for that and be concerned enough to secure them. When I am here, I try to secure
the faxes for Cecelia Huggins and put them in a folder in her office.

8. I was on leave Feb 19th and 22"and when I came back Tuesday, February 23, the fax machine was full of faxes.
Where was the concern then? Anything faxed or copied is accessible on a regular basis.

9. I am very concerned that someone would dig thru my desk drawers and find items to take pictures of. They took
pictures of patient information, which is unsecure? Were they reported by unsecure email? Isn't that illegal?
Invasion of privacy as an employee?
**The only picture (#2) that shows this is actually my desk, has no patient
information on or near it.

I am requesting a desk change, to somewhere that I have privacy. This atmosphere is getting more intimidating and the stress
is taking its toll on me. I am having to take leave on a regular basis, because of this stress and that is not fair.

We do not have patients come here, nor or we allowed to let anyone other than employees in. Also, there is a door bell that
can be rang should anyone need assistance at the front door.

I have a right to work with no fear but at this point, it will not be getting better.

This is rude and intentional.

On any given day, I don't know where anyone is, if a car is available, if it isn'

I am tired of this situation. I filed an EEO complaint against an "anonymous" employee who took
pictures of my desk in February, only to show I was "set up" under false pretenses. Nothing was
done about it nor is anything else being done. This is the type of people I work with and I know
management is very well aware of how they are. I have contacted management in writing for
many issues about everything going on here and how I feel humiliated, discriminated against and
scared in this hostile work environment. Things should not be like this. I am not even allowed to

situation at my office because patient care was being affected. I received no help from anyone.

At this point, the RN's were refusing to do their own care plans (and still do) and want me to do

Well, I guarantee you the nurses don't want to do anything they don't have to. Byron, the clergy is doing NURSING
care plans. ...... ..as per email below. I guess he does their "nursing" section but leave the "administrative" section of "review in
90 days" for me to do? John McBride isn't their supervisor and they very well let him know it. He can't make them do
anything that is for nursing. If you want nursing to do their own care plans, especially down here, people in Alexandria
should not have access to it on the NAZZ. That will never happen.

If one would look at the inter-d notes in the medical record for the past year, they would see they weren't being updated. Labs
a year old, dates of appointments not listed correctly, one date for BP taken but the BP listed is for another date, labs possibly
not listed when they should be, care plan stating the patient didn't fall but in reality the progress note states he did (actually 3
notes in 3 months state patient fell (it was cut/pasted word for word) - this is part of medical record and
JAHCO would close us down if they saw it there are LOTS of them that are incorrect and it is very noticeable, even to the
untrained eye.

I have reported this over and over again. As a "clerk", I should not be responsible for this, especially since I know what is going
on. Nursing doesn't want to address the issue with their own nurses (last time I said that, I received an ugly email from Uhla
Tademy about that). Every week they give John McBride a hard time about something that he has no control over. They won't
even read their own care plans in the meeting Alexandria HBPC seems to run smoothly but down here is a whole
different ballgame.

This isn't the only issue. We are still having trouble with the vehicles. I am asked to look for cars available for someone
(usually the Jennings CBOC) to borrow and I don't know who is here, what cars are available, if people will be using them,
NOTHING.

I can't tell ANYONE if our employees are out seeing patients, if they are on leave, if they are here._ .......... nothing. Now what
kind of safety is that? They think I am being nosey, wanting to know where they are at. In reality, I can't even do my
job It all boils down to no one wants anyone to know what they are doing that way they can come and go as
they please at all hours. It is just upsetting because I have Angela LeBlanc telling me yesterday that her needing a car is a $
issue and she has to do (whatever she is doing) so I am scrambling to find a car but am clueless as to anything about
them.

This has been going on for years and it is easier to ignore it than to address it. The employees will run to the union for
help no one makes these employees accountable, they always have an excuse.

The VA doesn't make nursing responsible for their own care plans OR their own travel issues. It has affected patient care and
it's bad.

I have reported NUMEROUS times everything going on here there is a wall hanging in my office that states to report
patient safety and quality of care concerns to the director. Well, that does no good because this has continued for years.
Last time I reported something, someone tried to set me up by taking pictures of my desk. It was an obvious set up and
nothing came from it but I guarantee you, it isn't over. These people steam roll over anything and anyone per an
employee once, "I will not be micromanaged".

I am sure someone will come after me for reporting this it's the norm. I wish the union would see how this situation has
created issues in patient care and make people accountable. I understand the union is here to protect our rights but
sometimes, things need to be addressed for the greater good.

Just so you know after I have reported numerous times that I am afraid, I am paranoid, etc., things here will never
change. I am still being treated for a massive ulcer, severe insomnia, depression, etc and it is work related. That is why I have
taken FM LA for 3 years and counting. It's a bad situation. It is also a sad day for our veterans.
Another email sent to the union for help. No help received.

Now let's see who is trying to deflect what is really going on???? I have"??" patient names/socials. Keep in mind,
HBPC is to see patients every 30 days. We are no more short of staff than anyone else, Alexandria HBPC seems to
run just fine. Some of these are for last summer, this is an accumulation of things. Notes are to be entered within
48 hours. This is just a small portion of what is really going on and these people are pointing the finger at me
or Mr. McBride, who isn't their supervisor. Has anyone wondered why these patients' medical records aren't
correct? Has anyone wondered why RN's are complaining they can't give out a calendar to let someone know
where they are because they might have a change in plans BUT one RN if talented enough to see 9 patients and
travel 260 miles before noon? I am responsible for the cars but ultimately don't have a clue as to where they are
unless they are in our parking lot — then, I don't know if they are to be taken out that day or any other day. I also
am unaware of where anyone in this program is whether they are off, out in the field or in the office. Should
a patient call, JCAHO come in the front door or someone call with a critical lab or emergency, I have no idea about
anything. It is very convenient that one would not want anyone to know where they are considering the
inaccuracies of the medical records.

There seems to always be opposing issues to anything that is suggested and in the end, it is affecting patient care in
less than a month we have discharged 6 patients and have admitted "0" I don't see anyone yelling, "give me one to
admit" and s/c vets waiting as long as 460+ days on the EWL. We are not up to our maximum of 30 patients/RN or 90/NP.
We are currently at 74 as of today and looking like we will be discharging another next week.

August 10 - at least 7 notes stating wellness phone calls were made to the patients July 30. These notes 10+ days late.

? 106 days between actual RN visits (but they will say, "we were short of staff, we were busy, we were told we could just have
wellness call made". No call or visit in June, call made in July but documented in August, attempted to see in August by making
phone calls but patient didn't answer.

? — 126 days between actual RN visits (wellness call was made in July??? (documented in August) and August, no call in June)

? — visit was 6/8/15 but inter-d note in system shows visit states inaccurate date of 7/8/15 as did the care plan (I had printed
copy). Patient wasn't seen in July

? - Careplan was due 5/20/15 but as of 7/28/15, it wasn't updated

? - 5/27/15 inter-d note entered 6/15/15 — 84 days between RN visits; no "wellness" call made June or July.

? — care plan updated 12/30/15 has old information from September.

? careplan for 1/6/16 has old information from October.

? careplan for 1/6/16 has old information from September.

? careplan for 12/30/15 has old information from October.

Also, no lab results are listed on #3 although there are numerous problems listed. (on numerous inter-d notes) ???

As per today, patient not screened again today on EWL 518 days

I happen to come across this because this patient came up for inter-d on Wednesday. If I am not mistaken, inter-d notes have
to be up-to-date should we get audited. It seems every time I happen to look at one, it is inaccurate or not updated.

9/23/15 inter-d note states "7/8/15 pain level is 0". The patient happened to call in to the nurse on 8/27/15 as documented in
record stating his pain level was 9/10. He didn't have an RN visit in August. The NP read the note 8/28 and a wellness call was
made 8/31/15 by NP.

**Pain med refill was due 7/31/15 (last fill 6/30/15) but patient evidently didn't call in to request it and since he wasn't seen in
August, no one caught it. He finally called 8/27/15 stating he was hurting real bad and could barely get out of bed. Pain level
was 9/10. Patient not seen by RN until 9/30/15. NOTE: The RN entered the note Thursday, 8/27/15 and signed it at 3:24 PM.
The HBPC physician works 8-4:30 (T, W, and Th) and did not see it until the Tuesday when he arrived back at work. He entered
it then. The NP, whose patient it is, read it on Friday, 8/28/15 and made a wellness call 8/31/15. She can't sign narcotics so
the patient waited until 9/1 for it to be ordered, when the doctor came back to work. The pain med was filled Tuesday, 9/1/15.

**patient was seen by RN 7/8/15 and 9/30/15.

**7/15/15 inter-d note has vitals from 5/5/15 although he was seen 7/8/15 and 6/24/15.
Many more care plans have not been updated so therefore, it appears even more inter-d progress notes are not noted with
up-to-date information. It is just cut and pasted from whatever is there. I have been told the RN's are supposed to be
updating care plans during the meeting but that is not happening, well, some are doing it correctly. They update them before
the meeting are prepared, some, well, they just go against the grain for everything.

***Evidently something is beiniLaddressed because I am feeling the pressure. I knew this would happen when I found
someone who may address the issue. I am sure the worst is yet to come. Thanks for all you are trying to do. I do appreciate it

Girl, I don't know about the name thing. I am going to think about it. I am afraid of these people. They are bullies. When I
reported things in January they did that picture taking thing "anonymously". (I have since fo9und out who it is) - I will send
you that as well. John McBride can't do anything with them. He isn't their supervisor. Nursing evidently just turns a blind eye
because things still aren't right. All I can say is, if people came to work and had to sign in and be accounted for, the patients
would get seen in a timely manner. I really don't care if if and when people come to work but don't sit there and whine about
not being able to see patients when you don't come to work. I wish I had a lap top so I could go anywhere and no one would
ever know where I am at.

Every meeting we have they want to go against the system We have rules and guidelines we have to follow but no one

Did you know I have taken FMLA for the last 3 years for stress, I am on my 11+ different prescription for sleep.. ..... doctor told
me I should get rid of my conscious then maybe I could sleep.

After speaking with you on Friday, I forgot to ask. Is there any way I can apply for reasonable accommodations to perhaps
move from this office to another office? It worked out fine when they moved me to another building because of a hostile work
environment filed on me (btw, nothing was substantiated, I proved it false).....I stayed there for 6 months and communication
was by email (or phone if need be).

We do not see patients here, nor do we have anyone come in and out. I know there is an office in the Jennings CBOC because
a RN just retired and the NP is now in Lake Charles.

My health is suffering I can get my physician to put that in writing it has been since at least 2011....I came to HBPC in
October, 2010. I've been hospitalized numerous times and after 5+ years, one would think my issues would be getting
better.

After 11 different meds for insomnia, plus many other health issues (depression, anxiety, chronic gastrojejunal anastomatic
ulcer, ulcer anastomatic, heart palpitations, real high cortisol level - stress hormone etc.), I need to do something. This is all
documented in my personal medical record I have claimed FMLA every year for the last 4? years when it is in reality, a
work related issue. It is obviously nothing is ever going to get better here

Please keep this between you and me at this point. I am just asking for an opinion ;-(

Just wanted someone to be aware I have had calls from Alexandria and Jennings labs with critical labs and never seem to
know where anyone is....provider, RN's, etc. This has caused a problem in the past and can cause a serious patient issue. I've
actually been yelled at by lab personnel because "I" couldn't find an employee who was supposed to be at work but nowhere
to be found.

There are days I need to find a vehicle for the CBOC to use but have no idea who is here and who isn't; who is on leave and
who isn't. I also have found out that people are not on leave BUT there are cars in the parking lot, they just aren't in the
building. When I loan the car out, then I am told, "I was going to use that car" but had no idea that anything was scheduled.
That has caused a problem when I am trying to track people down for different situations.

I have also had other VA employees call needing to speak to HBPC staff immediately here and have had to say, "I don't know
where they are at" which on one occasion, the employee thought I was being untruthful. They don't answer emails or
phone calls at times.

I have actually sent numerous emails to an employee re: patient care, not knowing they were on leave and waiting for a
response. This has prolonged patient care. I have also forwarded calls to employee voice mails, not knowing they were on
leave.

While it seems it was discussed to use a text system, I only have a personal cell phone. I am no longer going to use that for
business.

!just want someone to be aware it isn't just about having a "buddy" system; this issue is affecting patient care. I know they
are ultimately responsible but I also know how things go around here.
