MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT, made and entered into, by and between:

The COLLEGE OF VETERINARY MEDICINE AND BIOMEDICAL SCIENCES -


CAVITE STATE UNIVERSITY, a college operating through CvSU BOR Resolution
No. 50, s. 2000, with office at the Cavite State University, Don Severino de las
Alas campus located at Barangay Bancod, Indang, Cavite, represented herein by
its Dean, DR. ALVIN-WILLIAM A. ALVAREZ, hereinafter referred to as the
CVMBS-CvSU,

and

The BARANGAY ________________, with principal office at the _______


Barangay Hall, ________, Indang, Cavite, represented herein by its Chairman,
HON. _____________________, hereinafter referred to as BARANGAY
__________,

(collectively referred to herein as the PARTIES)

WHEREAS, CVMBS - CvSU has an established commitment to share expertise in


selected fields of study by way of extension services to its surrounding community;

WHEREAS, CVMBS-CvSU has designed an extension program known as


“Boluntaryong Beterinaryo Para sa Kabuhayan at Kalusugan or 2B@2K” which has
component projects that aim to promote responsible pet ownership, reduce rabies
incidence, and improve knowledge and provide technical assistance to small-scale animal
raisers;

WHEREAS, BARANGAY ____________, is fully committed to uplift the lives of its


constituents by ensuring a safe community and providing opportunities for improving
livelihood;

WHEREAS, the BARANGAY ____________, has signified interest to serve as a


partner of the extension program.

NOW, THEREFORE, for and in consideration of the foregoing premises and the
covenants and stipulations hereunder, the PARTIES have agreed as follows:

1. Component Projects and Activities of the Program for Implementation in the


Barangay:

Component projects and activities that will be implemented and regularly conducted at
the barangay are as follows:

1.1 Rabies Prevention Project

1.1.1 Free Rabies Vaccination for Dogs and Cats. This shall be conducted
annually and will target at least 80% of dog and cat population in the
barangay. This shall be conducted in coordination with the Municipal
Agriculture Office of Indang and other government agencies who have
similar mandates or interest.
1.1.2 Information and Education Campaign (IEC) on Rabies prevention and
Responsible Pet Ownership. This shall be conducted at least twice a
year and shall target all constituents of the barangay who owns a pet dog
or cat. This may include IEC material distribution and seminars on the
topics.

1.1.3 Orientation on Rabies and Animal Welfare Laws for Barangay Officials
and other personnel. This shall be conducted at least once during the
incumbency of the target officials and personnel.

1.1.4 Spaying and Neutering of Pets. This shall be conducted at least once a
year at the CVMBS-CvSU veterinary teaching hospital. The number of
pets that will be accommodated shall vary depending on the availability
of the needed materials, especially the amount of anesthetics that can be
provided. This shall be provided for free to the constituents but cost of
anesthetics may be shouldered by the barangay, the college, or a
sponsor.

1.2 Seminars on Small-Scale Animal Raising

1.2.1 Seminars for Small-Scale (Backyard) Animal Raising shall be conducted


either at CVMBS-CvSU or at the barangay. Topics may be requested by
the barangay, scheduled by the CVMBS-CvSU, and/or requested by
companies through the CVMBS-CvSU or the Barangay.

1.3 Technical Assistance for Small-Scale Pig Raisers

1.3.1 Technical Assistance shall be extended by CVMBS-CvSU to small-scale


(Backyard) Pig Raisers of Barangay ________. This shall still be needs-
based and shall be scheduled. The CVMBS-CvSU shall conduct needs
assessment in the barangay at least once a year and shall schedule
visits to individual raisers. Raisers who were not included in the needs
assessment may forward their requests to the barangay for collation and
forwarding to CVMBS-CvSU. Services shall be provided for free to the
constituents but cost of dewormers, medications, vaccines and other
supplies may be shouldered by the barangay, the college or a sponsor.

2. Responsibilities of CVMBS-CvSU:

2.1 The Extension Coordinator of CVMBS-CvSU shall serve as the focal person for
the college and shall primarily be responsible for coordination with the barangay.
2.2 CVMBS-CvSU shall always ensure that all activities are well-coordinated with
the barangay and that personnel, including faculty-veterinarians, staff and
students, are available for deployment during these activities.
2.3 CVMBS-CvSU shall furnish the barangay with a list of faculty-veterinarians who
will be involved in the activities which will be updated every semester or as
needed.
2.4 CVMBS-CvSU shall prepare budgetary requirements for all scheduled activity
per year, based on the approved 2B@2K program and make the necessary
purchases before the activities. CVMBS-CvSU may request the barangay to
prepare additional supplies that it may not be able to provide.
2.5 CVMBS-CvSU may invite a Third Party to participate in or sponsor activities.
This may include private companies or individuals, NGOs, other Government
Organizations, and student organizations. This shall be coordinated with the
barangay.
2.6 CVMBS-CvSU shall deploy student-clinicians to the barangay for long-term
activities such as IECs, needs assessment, and data collection necessary for
the conduct of activities and assessment of projects and programs.
2.7 CVMBS-CvSU shall conduct analysis of data collected from the barangays
which may be used as a research output of the college. Results of analysis shall
be cascaded to the barangay.

3. Responsibilities of Barangay ________:

3.1 Barangay _______ shall designate a Barangay Coordinator who shall primarily
be responsible for coordination with the extension coordinator of CVMBS-CvSU.
3.2 Barangay _______ shall always ensure that all activities are well-coordinated
with the CVMBS-CvSU and that personnel are available for deployment during
these activities.
3.3 A Special Order (SO) for faculty veterinarians shall be issued by the barangay.
3.4 Barangay ________ shall include, in their yearly budget, supplies and other
materials based on the request of CVMBS-CvSU and make the necessary
purchases before the activities.
3.5 Barangay ________ shall be responsible for the security of personnel, and
students of CVMBS-CvSU, and third party guests during the conduct of activities
at the barangay.
3.6 Barangay ________ may invite a Third Party to participate in or sponsor
activities. This may include private companies or individuals, NGOs, other
Government Organizations, and student organizations. This shall be coordinated
with the CVMBS-CvSU.
3.7 Barangay _______ shall accommodate and assist student-clinicians to the
barangay for long-term activities such as IECs, needs assessment, and data
collection necessary for the conduct of activities and assessment of projects and
programs.

4. Other Covenants/Stipulations:

4.1 Nothing shall diminish the full autonomy of either PARTY, nor will any constraint
be imposed by either upon the other in carrying out the agreement. Although
both parties have financial responsibilities based on this agreement, these shall
be treated as voluntary by either party and not an obligation to each other.
4.2 Lunch, snacks, and other expenses incident to the conduct of activities shall be
borne by either or both PARTIES.
4.3 All participants shall be obliged to religiously observe the policies, rules and
regulations, of Barangay ________ or CVMBS-CvSU while in the jurisdiction of
either PARTY.
4.4 Either PARTY shall not be held liable to any participant, or to any other party, for
any fortuitous events or bodily harm or injury that may result from the
participant’s own action(s) which is (are) not part of the activity.

5. Effectivity:

This MEMORANDUM OF AGREEMENT shall take effect upon signing by the


PARTIES and shall remain in full force and effect until the last term of the incumbent
Barangay Chair who is signatory. This agreement may be renewed or terminated earlier,
subject to such terms and conditions as may be agreed upon by the PARTIES, through a
Written Notice of intention to renew or terminate the same.
IN WITNESS WHEREOF, the PARTIES have affixed their signatures on this
