AFFIDAVIT OF EXPLANATION

I, KRISTINE KAYE ROMERO Y CULA, of legal age, Filipino, single, and

presently residing at Purok Sigay, Barangay 2, Bacolod City, Negros Occidental, after
having been duly sworn in accordance with law, hereby depose and say:

1. That I was born on April 29, 1987, at Pandacan, Manila, to CELSA D. CULA and
NOE M. ROMERO;
2. That my parents transferred residence from Manila to Bacolod City sometime
in 1988;
3. That my parents were married on August 15, 1993, six years after my birth (a
copy of their Marriage Certificate is herein attached);
4. That my Certificate of Live Birth reflects the fact that I was born out of wedlock
as evidenced by the surname that I carry (CULA) which is that of my mother
as appearing therein;
5. That my Certificate of Live Birth does not contain the name of my father NOE
M. ROMERO as my parents were not yet married at the time the entries in the
Certificate of Birth Form were filled out;
6. That I have been carrying the name KRISTINE KAYE C. ROMERO since birth,
and that the same name appears in all my records except my Certificate of
Live Birth;
7. That the name CRISTINE KAYE CULA as appearing in my Certificate of Live
Birth is the same as KRISTINE KAYE C. ROMERO, the latter being my true
and correct name as appearing in all my private and government records;
8. That I have two (2) other siblings born of my parents CELSA D. CULA and NOE
M. ROMERO, namely:
a. JOHN PATRICK C. ROMERO
b. NOE C. ROMERO, JR.
9. That I am executing this affidavit to attest to the truth of the foregoing
statement and for whatever legal purpose it may serve.
IN WITNESS THEREOF, I have hereunto affix my signature this ______ day of
