legal age, Filipinos, married, and residents of _________________, Balasan, Iloilo, hereby jointly
depose and say:

That we personally know JANETA ANDRDE GABRIEL, the daughter of the late Spouses
Leopoldo B. Gabriel and Asuncion B. Andrade, being our neighbor and close friend for a long time;

That JANETA ANDRDE GABRIEL was born on June 10, 1966 at Balasan, Iloilo and such facts of
birth were duly recorded before the Office of the Local Civil Registrar of Balasan, Iloilo;

However, relative to a CERTIFICATE OF LIVE BIRTH of MERJAN GABRIEL IGONG-IGONG from

the Office of the Civil Registrar General, Kalibo, AKlan, with BReN 00407-A99LTO1-8, it appears
thereon that her name in the field of mother is written as JANETHA ADRADE GABRIEL instead of
JANETA ANDRADE GABRIEL her true and correct name;

Likewise, relative to a CERTIFICATE OF LIVE BIRTH of JASMIN GABRIEL IGONG-IGONG from

the Office of the Civil Registrar General, Kalibo, AKlan, with BReN 00407-B03V203-6, it appears
thereon that her name in the field of mother is written as JANETH ADRADE GABRIEL instead of
JANETA ANDRADE GABRIEL her true and correct name;

That the name JANETA ANDRADE GABRIEL appearing in all her records and other official
documents and JANETHA ADRADE GABRIEL the name appearing in the CERTIFICATE OF LIVE BIRTH
of MERJAN GABRIEL IGONG-IGONG from the Office of the Civil Registrar General, Kalibo, AKlan,
with BReN 00407-A99LTO1-8; and JANETH ADRADE GABRIEL appearing in the CERTIFICATE OF LIVE
BIRTH of JASMIN GABRIEL IGONG-IGONG from the Office of the Civil Registrar General, Kalibo,
AKlan, with BReN 00407-B03V203-6, refers to one and the same person;

That such discrepancy in the entries was because of error/mistake in recording the first
name;

That we are executing this affidavit for the purpose of attesting to the truth of the above-
statements and for all legal purpose it may serve.
