AFFIDAVIT OF LOSS

I, RONNIE N. AGGASID JR., of legal age, single, Filipino

citizen, and a resident of Brgy. Roxas, Solano, Nueva Vizcaya, after
having been duly sworn to in accordance with law hereby depose and
state:

1. That I am a holder of pertinent documents issued by Southern

Isabela Colleges of Arts and Trades (SICAT) in claiming my
Certificates in Technical Education and Skills Development
Authority (TESDA);

2. That the said documents were always in my possession,

however, sometime in 2009, I noticed that they were missing
and despite earnest efforts, I failed to find them anymore;

3. That finding the futility of recovering the said documents, it is

hereby declared that the same are considered lost and beyond
recovery;

4. That I attest to the veracity of the foregoing statements.

IN WITNESS WHEREOF, I have hereunto set my hand on this

affidavit this 1st day of August 2017 at Bayombong, Nueva Vizcaya.
