MEMORANDUM OF AGREEMENT
(Program Contract)

The Department of Education, Regional Office –IX, herein referred to as “DepED RO – IX”
with principal office at Airport Road, Tiguma, Pagadian City, herein represented by

In consideration of privilege to avail of the Modular Program on official time with pay, do hereby
agree to the following obligations to the trainee:

1. Specialize in PROFESSIONAL DEVELOPMENT PROGRAM FOR SHS – TVL

TEACHER IN FISHERY ARTS IN THE DEPARTMENT OF EDUCATION IN
REGION IX

2. Shall attend to the program only during Saturday, provided that necessary arrangement of classes
be done with School Heads consent;
3. Maintain at least the minimum standards of the program and that failure to do would be sufficient
ground for disqualification in the event of which he/she shall be held liable by the government in
favor of his/her program;
4. Conduct himself/herself in such a manner as not to bring disgrace or dishonor for himself/herself,
his/her school/agency and the Department;
5. Submit to the Curriculum and Learning Management Division, Copy Furnish Regional Director’s
Office at the end of the program, a copy of his/her action plan for the Division;
6. Immediately report to his/her station and assume his/her function upon the completion or
termination of the program;
7. Teach grades 11 and 12 classes in the area where he/she was capacitated;
8. Shall serve his/her school/agency for at least three (3) years after the program;
9. Shall pay the One Thousand Five Hundred Pesos (Php 1,500.00) as Training Fee that will cover
payment for the honorarium of Trainers/resource persons, rental of equipment, and training
materials charged to their respective School MOOE;

 10. Shall refund to DepEd all expenses incurred on salaries and other benefits for failure to comply
with any of the foregoing conditions, through willful neglect, resignation from service, transfer to
other government agencies, voluntary retirement or other causes within his/her control. For
reason beyond the teacher’s control, the conditions do not apply, provided all documentary
requirements including the doctor’s certification are submitted and are acceptable to DepEd.

11. Shall live up to terms and conditions of the program.
