Dear Customer:

Thank you for agreeing to assist your Mary Kay Independent Beauty Consultant in her entry of the Mary Kay
Beauty ArtistTM Contest (“Contest”). Mary Kay (Malaysia) Sdn.Bhd and Mary Kay Singapore PTE LTD
(“Sponsor” or “Mary Kay”) will be managing the Contest using internet based tools. In order to participate in your
Mary Kay Independent Beauty Consultant’s creation of entry for the Contest, you will need to consent to certain
information being used for that purpose.

Your Mary Kay Independent Beauty Consultant (“Beauty Consultant”) will take two photos of you: one before
your makeover and a second after your makeover. Your Beauty Consultant will upload those photos onto the
Mary Kay Beauty ArtistTM Contest web site at www.mkdreambeautiful.com.my or
www.mkdreambeautiful.com.sg along with your personal information (including your name, address, email
address, and phone number), and this Consent Form (“Entry Materials” or “Entry”). Once the Entry Materials
are submitted, your Entry will be complete!

After submitting your Entry, your photo will be posted in the Contest web site Gallery of Entrants along with
your, name for a public voting stage. For a more detailed description of the Contest please refer to the Official

Also, because your privacy is important to the Sponsor, you have a choice with regard to what information you
choose to share with the Sponsor and Contest Entities (as defined below). Please indicate your consent below
to allow your Beauty Consultant to share your photos, personal information, and email address with the Sponsor,
its subsidiaries and affiliated companies, contractors, and agents and those individuals/entities engaged in the
development of, the production or distribution of materials for, or the implementation of this Contest (“Contest
Entities”). The Sponsor and Contest Entities will only use the information you provide for the purpose
of conducting the Contest as described in the Official Rules and will not sell or otherwise disclose your
information to any third party.

writing Mary Kay (Malaysia) Sdn.Bhd and Mary Kay Singapore PTE LTD or sending an email.
