AFFIDAVIT

I, PO1 MARISOL SUZETTE M. PELONGCO, a Police Officer, of legal age,

single, Filipino, with residence at Isabela City, Negros Occidental, after having been
sworn to in accordance with law, do hereby depose and say:

1. That I am currently assigned at the Bacolod City Police Station;

2. That on January 1, 2018 at about 8:30 in the evening, I asked permission

to PSI Wilfredo Benoman that I will be absent on January 2 for my pre-
natal check-up at Isabela Rural Health Unit, Isabela, Neg. Occ.;

3. That when he asked when am I coming back, I answered that after my

check-up I will be on duty immediately;

4. That on January 2, 2017, I was surprised to know that all check-ups were
canceled because it was a non-working holiday, prompting me to decide
to wait until January 3, 2018 for my check-up;

5. That on January 3, 2018, I arrived at SM North Wing at about 3:00 PM,

after I came back from my pre-natal. My colleagues said that the team
in-charge of attendance has just passed in their area;

6. That I did not intend to commit such absence in my duty, and promise to
do my best to avoid committing the same.

7. That I am executing this affidavit to explain why no disciplinary action

should be taken against me and to attest to the truth of the foregoing and
for whatever legal purposes that this statement may serve.
