Role of Kushthae Thila Kalang in the treatment of Wajul mufasil

(Rheumatoid arthritis) – A Review

*Background
Rheumatoid arthritis is an autoimmune disorder. According to the Unani concept it mainly caused by
derangement of humoural temperament and accumulation of fasad mavatha in the joint spaces..
Prevalence is 1 % in word population.

The Kushthae Thila Kalang is finest form of powder used as medicine prepared by process of
calcination of the metal, mineral and herbal origin drugs. Specially it contains “Thila – Gold.

*Aim
Even several treatment methods are available; the percentage of recovery is low. So, the aim of
the study was to review the efficacy of Kushthae Thila Kalang in the treatment of Wajul mufasil.

Conclusion
Several in vitro studies have been demonstrated immune modulatory action of this Kustha
preparation. Based on the available data, it can be concluded Kushthae Thila Kalang plays an
important role in treating Wajul mufasil. Hence more applied research need to be conducted in
this regard.

Abstract
Kushta Tila Kalan (KTK), a gold preparation used in Unani-Tibb is claimed to possess general tonic,
anti-infective and rejuvenating properties. We evaluated immunomodulatory activity of KTK in male
mice. KTK was orally administered to animals at dosage of 6.25, 12.5, 25 and 50 mg/kg b.w. for 10
days. Beside general immunopathological parameters, cell-mediated immunity was evaluated by
measuring delayed type of hypersensitivity response (DTH) while humoral immunity was assessed
using plaque forming cell (PFC) assay. KTK augmented both the immune responses at dose levels
of 6.25, 12.5 and 25 mg/kg. The optimum activities were recorded at 25 mg/kg. High dose of 50
mg/kg showed suppressive effects on immune functions. The modulatory effects may be attributed
to the interactions of gold with herbomineral adjuncts incorporated during the specialized ashing
techniques used in the preparation. The results are interesting in view of reported suppressive
effects of other gold preparation

Rheumatoid arthritis treatment


One of the earliest treatments for rheumatoid arthritis, gold therapy, a disease-modifying anti-
rheumatic drug (DMARD), has declined in terms of use over the last two decades due to the
arrival of better tolerated biologic and non-biologic DMARDs, such as methotrexate.

The most common form of gold treatment is via injection while the oral gold formulation appears
to be less effective. Patients usually receive weekly injections which may eventually reduce to
once a month.

According to a review, people with this condition receiving gold injections had 30% fewer
swollen joints compared to people receiving placebo. The authors concluded that “although its
use can be limited by the incidence toxicity, injectable gold has an important clinically and
statistically significant benefit in the short term treatment of patients with rheumatoid arthritis.”

Jinoos Yazdany, associate professor of medicine at the University of California in San


Francisco, explains, “The efficacy and side effects of the newer medications are so good at
slowing down the course of the disease that other drugs should be considered first before gold.”
