Maguikay, Mandaue City, after being sworn to in accordance with law, depose and
say that:

1. I am authorized by EMMANUEL P. CARREON to receive his

employment salary;

2. Attached herewith is his authorization letter, which shall form part of

this affidavit;

3. I am aware that I may be held accountable for perjury for any false
statement/s made in this affidavit;

4. I am executing this sworn statement, as required, to attest to the truth

of the foregoing statement; and
