INSPECTION / SURVEILLANCE FLASH REPORT

1) DATE / PERIOD Of INSPECTION: 11th June 2018

2) DETAILS OF WORK PERFORMED & RESULTS:

 Follow up with convergent to check the status of Dampers and sand filters and
status of delivery of ducts 1A. Vendor informed dampers and filters will be ready
on 12th June, ducts of 1A was partially delivered to KSS and remaining ducts will
be delivered on 12th June 2018. Instructed KSS to start the painting activities in
order to meet the schedule.

 It was noticed some ducts are not insulated due to un availibity of insulation
material at KSS, discussed with Convrgnt to arrange the material for insulation
to start the activities. Instructed to start the packing activities.

 Packing activities were started in the afternoon, packing list was prepared by
TCM and no support from celantel/Convrgnt on preparation of packing lists.

 Monitored the packing activities of ducts.

 COC was not provided by vendor for packing list-17.

 MTC’s shall be provided by vendor.
