MINI TASK 1

Rationale: In this activity, you are to interview a person (any) whom you know has acquired or still
experiencing any circulatory-related diseases. This activity will be essential and at the same time
fundamental for you to come up with your final output which later-on will be announced further
leading to your First Quarter Performance Task. For now, kindly ask the following questions and
make sure to take pictures for documentation purposes.

INTERVIEW QUESTIONS:

1. What type of Cardiovascular Disease or Circulatory-Related Disease have you had

(stroke, heart attack, blockage of blood vessels, chest pain, etc.)?

2. What were the signs and symptoms you experienced?

3. What kind of treatment did you have/undergo (surgery, medication, etc.)?

4. Did your doctor suggest that you make any changes in your habits such as your diet
or physical activity? If so, please explain.

5. How has Cardiovascular Disease affected your life?

6. If given a chance, what could you have done in order to avoid getting the disease?
Any message to the young who have not acquired yet the disease? What to do to avoid?
