AFFIDAVIT OF LOSS

I, REY FRANCIS U. ALTAREJOS, Filipino, of legal age, and a resident of No. 28

12th Ave., Murphy, Cubao, Quezon City, after having been duly sworn in accordance
with law, hereby depose and say:

1. That I am the owner/possessor of high school diploma for the school year of 1981-
1982 issued by Camp Gen. Emilio Aguinaldo High School in Murphy, Cubao, Quezon
City;

2. That sometime in January 2018, I was looking for the above mentioned high school
diploma, but to no avail;

3. That despite diligent efforts to locate the same all efforts proved fruitless up to the
present and fear that the same was lost beyond recovery.

4. That I am executing this affidavit to attest to the truth of the foregoing instrument and
for any legal purposes and intent this may serve.

Re: Request for copy of High School Diploma

Greeting!

Sir/Madam:

I would like to request in your good Office for the issuance of anew in lieu of said
lost one of high school diploma for the S.Y. of 1981-1982 (Section : Molave under
Adviser Mrs. Dolly Adarna Canonse).

This request is in connection as requirement for my application for employment

abroad and any other purposes this may serve.

Any assistance to be extended for the issuance of said request copy of above
high school diploma will be high appreciated.
