Guest Advise Form

* Completed Guest Advise Form must be submitted two (2) working days before guests' arrival for approval.
* The Unit Owner/ SPA must provide an on-site Contact Person, Name and Contact Number for their guests to contact upon arrival and for
the duration of their stay in Azure. Unit Owner/ SPA is responsible of informing their guests that their presence at Azure is subject to the
House Rules and Regulations of AURCC.
* The Unit Owner/ SPA must attach herewith a photo/scan copy of their visitors'/guests' Passport for Foreigners, Gov't Issued ID for locals.
* Upon proper accomplishment of the Guest Advise Form, the Unit Owner/SPA will be issued two (2) approved copies by the PMO, one
(1) copy for the Unit Owner/SPA and one (1) copy for their Visitor/Guest Representative. It will be the reponsibility of the Unit Owner/SPA
on how to provide the copy of their Visitor/Guest Representative before their arrival two (2) days after for them to present to this to the
Main Entrance Gate on the day of their arrival.
* On the day of the arrival of the Unit Owner's/SPA's Visitor/Guest at Azure, they will then present the approved Guest Advise Form at
the Main Entrance Gate to be allowed entry to Azure. NO APPROVED GUEST ADVISE FORM PRESENTED BY GUEST, NO ENTRY.
* For Unit Owners who own a carpark slot and wish to authorize their visitors/ guests to utilize the parking slot, please indicate your
park ing slot number and write the instructions in the remarks section below.

All visitors/guests are to abide by the Building Policies and Procedures and AURRCC House Rules and Regulations. Violators of any of the
provisions stipulated in the said documents will be penalized and are to pay the corresponding penalty fee. The Management reserves the
right to refuse entry and/or escort erring guests out of the Property. The Developer / AURRCC / Property Management Office shall not be
liable for any untoward incident, accident, injury, loss of property, or lives that may arise from the guests' stay in Azure Urban Resort
Residences.
