Subject: Closing of MDA Gate Pass 2000004151,Dt.25/01/2014.

NEW Fire Engineers Pvt. Ltd is a vendor appointed by BHEL to install CO2 Flooding system at CPP
Project, during commissioning it is found one of CO2 Cylinder’s Valve damaged, the cylinder was sent
to M/s New Fire Engineer’s Pvt. Ltd with returnable gate pass MDA Gate Pass 2000004151, Dated
t.25/01/2014. After continuous follow up the cylinder was not repaired and not received by KRIBHCO.
Later on it was decided by KRIBHCO, BHEL has to be back charged for the amount of cylinder and
BHEL has confirmed to KRIBHCO Project department email dated 29/05/2017 (attached) for the same.
KRIBHCO has issued PO, dated 17.08.2017 (attached) to New Fire Engineers Pvt. Ltd for procurement
of 68 ltrs. water capacity CO2 cylinder filled with 45 kg CO2 gas with master cylinder valve with flame
proof solenoid actuator, the vendor M/s New fire engineers pvt ltd will supply the material shortly.

So the returnable gate pass for sent CO2 cylinder may be closed in the record.
