Employee Information Usage Agreement

BYU-Idaho considers maintaining the security of confidential information a matter of highest


priority. As a condition of continuing employment, all employees granted access to such
information must agree to the standards set forth in this Employee Information Usage
Agreement. Therefore, I agree to:

1. Abide by the BYU-Idaho Computer Use Policy, including prevention of unauthorized


access to or use of any information in electronic data files maintained, stored, or
processed by BYU-Idaho by using safe password protocol, by protecting passwords, and
by locking or logging out of computer systems when leaving work areas. I also
understand that pursuant to the Computer Use Policy, BYU-Idaho may monitor use of all
University computing systems.
2. Disclose confidential data only to those authorized to receive it with the understanding
that portions of information made available to me, irrespective of the medium, may be
sensitive or confidential.
3. Respect personal privacy and comply with applicable law and University policy
governing the use of confidential information by utilizing, exhibiting, and divulging the
content of any record or report only as required to fulfill work assignments.
4. Ensure that any printed material containing confidential information is handled, stored,
and disposed of in a way that protects confidential information from unauthorized
disclosure.
5. Report any violations of BYU-Idaho security policies or attempted unauthorized access
of confidential information to an institutional compliance officer or other designated
agent of BYU-Idaho.
6. Ensure that third party contractors who are given access to any confidential information
execute a non-disclosure agreement available through BYU-Idaho Purchasing, which
requires the third party to abide by appropriate privacy and security standards.

I understand that my obligation under this agreement will continue after termination of my
employment. By signing this agreement, I agree to protect the security of confidential
information and maintain all such information in a manner consistent with the requirements
outlined in applicable law and University policy. Any breach of this agreement will subject me
to sanctions, including disciplinary action, under policies of BYU-Idaho as well as any
applicable law. By signing this agreement, I agree that I have read, understood and will comply
with all the conditions outlined in this agreement.

Work-Related Injuring or Illness Reporting Procedure

The following procedure must be followed whenever full-time, part-time, temporary, and student
employees are injured on the job or become ill due to their work responsibilities as defined in the
University’s WORKER’S COMPENSATION policy statement:

1. The employee must notify his/her supervisor immediately of all work-related injuries or
illnesses, regardless of the apparent severity.

2. Employees who sustain a serious or life threatening injury or illness should seek
treatment at the Madison Memorial Hospital emergency room. All other work-related
injuries or illnesses must be treated at the BYU-Idaho Health Center during normal
business hours. If the Health Center is closed, employees may seek initial treatment from
Madison Memorial Hospital or Community Care.

3. Employees who work or travel outside the Rexburg area and sustain a work-related injury
or illness should report directly to the nearest available hospital or general-care medical
facility if treatment is required.

4. Treatment from medical facilities other than those listed above and from specialized
medical professionals such as ophthalmologists, chiropractors, and physical therapists
may occur but only after a referral from the initial treating physician.

5. In any event, the University Security and Safety Office must be notified as soon as
possible so proper documentation can be promptly prepared. A report form must be
completed for every work-related injury or illness regardless of severity. This report form

Employees who receive treatment from medical specialists without a referral from a designated
provider as indicated above will be responsible for paying the medical bills for the non-referred
treatments, unless otherwise indicated by the University’s claims adjuster.

Sexual Misconduct Policy (Overview)

GENERAL POLICY STATEMENT

BYU-Idaho is committed to promoting and maintaining a safe and respectful environment for
the campus community. The university prohibits sexual misconduct in all of its forms,
including (but not limited to) sexual harassment, sexual violence, domestic violence, dating
violence, stalking, sexual exploitation, or indecent exposure (collectively "Sexual
Misconduct") perpetrated by or against university students, university employees, participants
in university programs, or visitors to its campus, whether the behavior occurs on or off
campus.

The university will take immediate and appropriate steps to stop Sexual Misconduct, prevent
its recurrence, and address its effects. Any person who violates this Policy may be subject to
discipline up to and including termination of employment, expulsion, suspension, or a ban
from campus, depending on the circumstances and the severity of the violation and the
violator's status as an employee, student, or visitor.

WHERE TO REPORT

Any person may report Sexual Misconduct to the Title IX Coordinator, 290 Kimball Building,
(208) 496-9200, titleix@byui.edu. The Title IX Coordinator oversees all Title IX complaints
and investigations, coordinates the university's response to sex discrimination, and provides
any person who reports Sexual Misconduct a written explanation of their rights, options, and
available services both on and off campus (including counseling, mental health, victim
advocacy, legal assistance, and student financial aid). Reports involving employees may also
be reported anonymously by using the BYU-Idaho Online Compliance Hotline. If a Report
involves a student or employee in a dual role (e.g. student employee), the Title IX Coordinator
will determine which office will investigate the Report. A Report of Sexual Misconduct
("Report") can be made verbally or in writing.

If a report of Sexual Misconduct involves potentially criminal acts, the victim should be
informed that he or she also has the option of reporting the incident to the Rexburg Police
Department at 208-359-3008 and the option to be assisted by campus personnel in notifying
such authorities. The victim should also be informed that he or she has the right to decline to
notify such authorities.
