    LOAN AGREEMENT OF PROFESSIONAL FOOTBALL PLAYER “THIS AGREEMENT (Agreement") is made on 14 June 2018 BETWEEN: (@ SPORT LISBOA E BENFICA ~ FUTEBOL, S.A.D., whose registered office is at Esadio do Sport Lisboa e Benfica, Avenida Eusébio da Silva Fencira, Apartado 4100, 1500-313 Lisbon, Porrugsl, tax number 504.882.066, hereinafter refered to as “BENFICA”, hereby represented by the undersigned Directors; () NOTTINGHAM FOREST FOOTBALL CLUB LIMITED (company registration ‘umber 01630402) of City Ground, Nottingham NG2 SJ; hereinafter referred to as “NOTTINGHAM FOREST”, hereby represented by the undenigoed members of the Bond, (© DIOGO ANTONIO CUPIDO GONGALVES, Portuguese Citizen, bom on 6 February 1997, bearee of the Portaguese ID a 13382120022 and valid uot 16 September 2020, with professional addvess at Rua Jozo Gouvels, 3 Bloco B R/C D,. Scisal, Portugal (bescinafer referred to as: PLAYER) WHEREAS A. BENFICA aad the PLAYER have eatered into « sports labour cootraet pursuant to the Jaws of Portugal 1B. NOTTINGHAM FOREST wishes to temporally acquire the right to the spocts performance and registration of the PLAYER; (C. BENFICA bas agreed to transfer tempocasily to NOTTINGHAM FOREST, subject to the conditions and terms hereinafee specified; D ~The PLAYER has agreed to transfer temporuily to NOTTINGHAM FOREST, subject to the conditions and terms hereinafter specified; and E ~ It's acknowledged chat NOTTINGHAM FOREST and the PLAYER have entered into a Deed in relation to the potential exercise of the Option and have agreed the terms of a fe furuce playing contaact which shall rake effect in the eveat that NOTTINGHAM FOREST exercise the Option All dis being stated, the partes hereco agree and sSpulate the Following And 1L4. The recitals constitute an integral, esseatial and binding part of this Agreement. Are 2 2.1, BENBICA temporal transfers to NOTTINGHAM FOREST (the sight to the spot ‘performance aod registration of) the PLAYER from 1 July 2018 unc 30 june 2019 (ihe “Loan Period"). 22.No loan fee shall be payable in respect of the temporary transfer of the sport performance and registration of the PLAYER from BENFICA to NOTTINGHAM FOREST. 2.3, For the Loan Pesiod, NOTTINGHAM FOREST shall be exchasively responsible for the payment of any salary dus to the PLAYER, in the terms agreed in the employment coatrrct between the PLAYER and NOTTINGHAM FOREST. BENFICA shall not, be liable for aay responsibilty concerning che employment contract signed between NOTTINGHAM FOREST and the PLAYER. 2.4, The PLAYER expressly and irrevocably accepts the content of this Agreement and, in particular his temporary transfer to NOTTINGHAM FOREST for the Loan Period. 28. The PLAYER expressly confirms that he has nothing to claim from BENFICA concerning any wapaid salary or bonus due until the signature of the present loan from the labour contact concluded between the PLAYER snd BENFICA, ot any due compensation conceming the preseat loan. Moreover, the PLAYER expressly authotises BENFICA to cance! his registration in Liga Portuguesa de Futebol Professional and Federngio Portuguesa de Futebol. The PLAYER ao recognises that during the Loan Pesiod BENFICA shall not pty acy payment (salary, bonuses oc whatsoever) to the PLAYER (for the avoidance of sny doubts, BENFICA will not pay any salasies or ‘omuses to the PLAYER regarding the Loan Pesiod), 2.6, BENFICA will lodge all necessary documentation with the Partguere Football Federation, snd shall perform any aecessary activity purruant to the FIFA TMS System, to allow the PLAYER’s temporary transfer to NOTTINGHAM FOREST until 30 June 2019. 2.7. NOTTINGHAM FOREST will lodge all necessary documentation with The Football Association and EFL, and shall perform any necessary activity pursuant to the FIFA TMS System, to facilitate the PLAYER’s temporary transfer to NOTTINGHAM. FOREST 2.8, Ifthe PLAYER suffers any injury dusing the loan pesiod NOTTINGHAM FOREST is obliged to inform within 48 (fory eight) bouss BENFICA’s medical department by e- sail to (luis Tl - uistl@slbentica pt. NOTTINGHAM FOREST shall bear all the necessary employment and health insurances for the PLAYER, NOTTINGHAM FOREST shall be responsible to pay all the PLAYER's medical treatments or operacions needed duting the Loan Petiod, NOTTINGHAM FOREST shall ensure that the PLAYER is segstered with the Premnies League Medical Health Care Scheme for the Loan Pesiod, 29. Dusing the Loan Pesiod, NOTTINGHAM FOREST stall eater into and maintain 2 policy with standird pre-conditions or exclusions, and at its own expense with = reputable insurance company insuzing the tisk of the PLAYER’s accidental death (aot asisig through eatural causes) and the ssk of the PLAYER suffering permanent total
