Requesting Medication and Supply Assistance   

                                                                For Medical Mission Trips  
 
Criteria for Request:  The request for St.Vincent to provide medications and/or supplies for 
international medical mission trips for an associate or physician must be provided on formal 
letterhead of the agency, church or group sponsoring the trip. Accommodations may be 
considered for local medication and supply requests. 

1. The St.Vincent associate or physician must be traveling with the organization on the 
mission trip. 
2. Please provide all of the following in your letter: 
 The name of the St.Vincent associate or physician participating in the mission trip 
 Purpose of the mission trip 
 Sponsoring organization 
 Destination of the mission trip including city/town, country and the local mission 
organization name 
 Groups / individuals participating in the mission trip 
 Dates and duration of the mission trip and the date that the medications or supplies 
are needed 
 Telephone number and email address so you may be contacted when the order is 
complete. 
3. Associates can submit only one request per year. 
4. If multiple associates are volunteering on the same trip, only one order will be filled.

6. St.Vincent Mission Integration Executive Director Loraine Brown approves these 
requests. 
7. Upon receiving your letter, a medication and/or supply list will be emailed to you.  
Please prioritize the items in order of importance for your trip. 
8. Due to the high volume of supply requests, your order can take up to two weeks from 
the day we receive your itemized list; with no exceptions.   
9.  You will be notified via email when your order can be picked up. 
 Medications:   St.Vincent Indianapolis Hospital, second floor Pharmacy  
 Supplies:  St.Vincent Indianapolis Hospital warehouse is located in the basement 
by the mailroom.

7048. 
 
A total donation of $500 is available per mission trip request.  For all Haiti mission trips a $1,000 total donation is available.
