AFFIDAVIT OF UNDERTAKING
That I, EVA P. HERNANDEZ, of legal age, Filipino, married, with residence
address at 154 Katipunan Avenue, St. Ignatius Village Quezon City, after having
been duly sworn to in accordance with law hereby DEPOSE and STATE:

1.) That I am the representative of HOMEOWNER’S

DEVELOPMENT CORPORATION with official business address at
154 Katipunan Avenue Street, Ignatius Village, Quezon City;

2.) That my principal owns an Isuzu NKR Truck with

PLATE NO.; OR NO. 1025211272; under MV FILE NO:1301-
00000758564; ENGINE NO. 4HG1498190A; MAKE: ISUZU; MODEL
NO; 2016; CHASSIS NO. PABNKR71E1G003G90; CR. NO.:
236928473; BODY TYPE: DROPSIDE

3.) That the above-described vehicle was involved in an

incident at Sorsogon City. That said incident was reported and
recorded at the Sorsogon City Police Station, CPS PNP under Police
Blotter Entry No. 0644 3.) That the vehicle involved in the said
incident was temporarily impounded by the local police authorities
for security reasons, for which I am appealing from the local police
authorities for the IMMEDIATE RELEASE of said vehicle, subject to
the condition that I will present or cause to be presented the subject
vehicle should the proper authorities would require it;

4.) That I hereby undertake that I will present or cause to be

presented the said vehicle should the court or any other authorities
require it for further investigation and/or legal inquiry;

5.) That I am executing this affidavit to attest to the truth of

the foregoing facts and to support my appeal from the local police
authorities for the immediate release of the subject vehicle;
