Filling in the IELTS

Application Form

Centre stamp: Leave blank. The appropriate British Council office will stamp this box.

Candidate’s recent photograph: Attach two coloured passport size identical photographs with out eye
glasses (not older than six months). Please refer to page iv of the IELTS Application form for the
specifications.

01. Preferred Date of Test:


Pencil in the first 2 dates of your choice which the appropriate British Council office is
offering. For details please visit British Council IELTS Test Calendar.

02. Last (family/surname) Name:


Enter your family name/surname/father’s name/last name (this is your surname as is
applicable). Please enter this name exactly as it appears on your Passport/CNIC.

For example: If your full name is Hassan Malik your family name will be Malik
If your full name is Muhammad Usman Khan your family name will be Khan
If your full name is Syeda Fatima Bokhari your family name will be Bokhari

03. Circle the appropriated title. Qualified doctors must circle only Dr.

04. First (given) name(s):


Enter your first name (followed by your middle name, if you wish). Please enter these
names exactly as they appear on your Passport/CNIC and must be in the same
order.

For example: If your full name is Hassan Malik your first name will be Hassan
If your full name is Muhammad Usman Khan your first name will be Muhammad Usman
If your full name is Syeda Fatima Bokhari your first name will be Syeda Fatima

05. Address for Correspondence:


Please ensure the correct mailing address is given, since your venue letter and result will
be sent to this address.

06. Telephone/Mobile Number:


Write your complete city code followed by your telephone number. You may also provide
your mobile number.

07. E-mail:
Please write your active e-mail address in clear handwriting.

08. Date of Birth:


Day first, month second and year in the last, all in numerals (e.g. 09/11/80) Please enter
your date of birth as it appears on your Passport/CNIC.

09. Gender:
Circle as appropriate.

10. Passport or Computerised National Identity Card Number:


Please indicate which document you will be using as a proof of identity, mention the
number and attach a photocopy. You must bring the indicated ID document on the day of
the test and that will be the only document accepted on the day of the test. Id documents
should be valid on the test day.

11. Nationality:
See the code list on page ii of the IELTS Application form. For Pakistan the code is 149.

12. First Language:
See the code list on page iii of the IELTS Application form. Your first language is your
mother tongue.

13. Occupation Sector and Level:


Look for closest description of your occupation sector and Level on page iv of the IELTS
Application form and put appropriate number in the boxes.

14. Why are you taking the test:


See the code list on page iv of the IELTS Application form.

15. Which country are you applying to: Tick as appropriate.

16. Which IELTS test module is you taking:


Tick as appropriate. Doctors proceeding to the UK for the PLAB test must take the
Academic module. All students going for further studies must also take the Academic
module. Potential immigrants to New Zealand, Australia and Canada may opt for the
General Training module. It is advisable to check with the university/ institute/ immigration
authority where you have applied, to confirm which module of the IELTS test they require
applicants to take.

If your answer to question 17 is ‘No’, please skip question 18. If it is ‘Yes’, please refer to
the Test Report Form of your previous attempt and fill in the exact details. Please also
attach a copy of the Test Report Form of your previous attempt with the IELTS Application
form. If this is your third or more attempt, please submit the copy of Test Report Form of
your most recent IELTS test.

Name of the academic institutes/government agencies/professional bodies/employers you
would like your result sent to. Result will not be sent to migration or education agents.

Declaration:
Please read the declaration on page 4 of the IELTS Application form and don’t forget to put
your signatures and date. Please note that if the declaration is not signed, your application
will be considered as an incomplete form and will not be processed further.
