Discrepancy in the surname and given name.
e.g. "Amelia" and "Susan", "Reyes" and "Perez"
Discrepancy in the date of birth
DOCUMENTS WITH MAJOR ERRORS SHALL NOT BE ACCEPTED.
MINOR ERRORS
Inconsistencies in the usual writing or printing of names of persons such as:
a. "MA" and "MARIA", "JR" and "JUNIOR"
b. "SIS", "SISTER" and "SOR"
c. "DE LA" and "DELA", "DE LOS" and "DELOS"
d. "MARYJANE" and "MARY JANE"
e. Names with or without hyphen
e.g. "JAY AR", "JAY-AR"
MINOR ERRORS
Wrong gender
, "Male" instead of "Female" and vice-versa
No gender printed on the birth certificate
Discrepancies in vowels/double and single letters in the spelling of first, middle and last name
, such as
"RAYMUNDO" "REYMUNDO" ,
"MUNDA" "MUNDO" "MONDO"
"BARRAMEDA" "BARAMEDA" and the like
Those identified as minor errors in the usual writing or printing of names, with or without hyphen,
whole or separate names, whole or abbreviated traditional names shall be accepted,
provided an affidavit of discrepancy shall be executed and submitted
.
Those wrong gender or without gender printed on the birth certificate shall be accepted provided all of
the following shall be presented and submitted:
a.
Baptismal certificate from parish
b.
Parent's or guardian's affidavit
c.
Certification from Brgy Chairman
or
his representative

Attend to your personal needs before the start of the exams in every subject. No examinee will be
allowed to go out of the examination room while the exam is in progress.
Stage 3:

1. Check if the Serial Number of Examinee Identification Sheet/Answer Sheets are the same for all the
sheets. If there is any discrepancy, return the set to your Room Watchers for replacement.

The Serial No. is NOT the examination number. It has nothing to do with your exams.
2. Check if there are defects or unnecessary marks on your Examinee Identification Sheet / Answer
Sheet
3. Check if the number of Answer Sheets corresponds to the number of examination subjects.

1. Use standard No. 2 pencil only
2. Do not apply too much pressure.

HANDLING OF TEST QUESTIONS SETS
1. Indicate your seat number at the right top corner of page 1.
2. Check if the number of pages of your Test Booklet is complete and no misprint. If there is any
problem, return the Test Booklet to the Proctor for replacement

3. Mark A or B on the answer sheet to indicate the set of Test Question assigned to you.
4. You can use your Test Booklet as your scratch.
5. Keep the Test Booklet stapled until the end of the exam.
PROHIBITED ACTS INSIDE THE EXAMINATION ROOM
1. Accepting or receiving anything, including food from any person while the examination is in
progress.
2. Giving money, food, or any favor and other consideration to the Room Watchers and other
examination personnel.
3. Loitering, talking, or discussing your answers inside the room or along the corridor while the
examination is in progress
4. Putting any of the following markings on your answer sheets: name, seat no., unnecessary words or
phrases, strokes, dots or any other marks not called for.
5. Taking out of the examination room test questions used or pages thereof, copying, and/or divulging
or making known the nature or content of any examination question or answer to any individual or
entity.

PROHIBITED ACTS INSIDE THE EXAMINATION ROOM


6. Copying or referring to any solution, answer or work of another examinee or allowing anyone to
copy or refer to your work, helping or asking help from any person or communication with anyone by
means of words, signs, gestures, codes and other similar acts.
7. Bringing inside the examination room books, notes, review materials, and other printed materials
containing principles or excerpts thereof, coded data/information/formula which are relevant to
connected with the exam subject and
PROGRAMMABLE CALCULATORS, CELLULAR PHONES, or other similar gadgets / devices. The
act shall be considered cheating and shall be ground for cancellation of exams.

PENALTIES FOR MULTIPLE MARKINGS & PLACING OF NAME IN THE PLACE PROVIDED
FOR THE SUBJECT OF THE EXAMINATION

1.
Placing Name
–Cancellation of the exam papers & suspension from taking the licensure exams for one (1) year.
2.
Multiple Markings
- Cancellation of the examination papers & suspension from the licensure examination for two(2)
years.

Use of correction fluid, sno-pake and other erasing agent is strictly prohibited.
STAGE 4:
CORRECTION, RATING & RELEASE OF EXAM RESULTS

READING BY OMR
The answer sheets are fed into the Optical Mark Reader (OMR)
CORRECTION
Checking/ correction by a computer program. Answer Key VS Marked Choices of the Examinee. NO
HUMAN intervention.

RATING
Once the ID Sheets are opened, all the staff involved in the release of results including the board
members are not allowed to leave the Rating Division until the results had been released to the press to
use the telephone or cell phone
RELEASE
Release of BOARD MEMBERS and Personnel immediately after copies for press release are brought
to the newspaper publications. NO RE-CHECKING. THE RESULTS, WHEN RELEASED TO THE
PRESS, ARE FINAL.

Stages in REGISTRATION and LICENSING

1.
OATHTAKING AS PROFESSIONAL

all successful candidates shall be required to take a professional oath to any member of the Board or
any office of the Commission prior to entering into the practice of PROFESSION

PAYMENT OF PRESCRIBED FEES
3.
ENTRY OF NAME IN THE
REGISTER OR ROLL OF PROFESSIONALS

4.
ISSUANCE OF REGISTRATION CERTIFICATE OR PROFESSIONAL LICENSE.
Upon payment of the registration fees, you will be issued the Board Certificate and the Professional
Identification Card.

Registration fee – P600.00


Annual fee (2015-2018) –P450.00
