AFFIDAVIT OF QUITCLAIM

I, ANNABELLE C. ENAD, of legal age, married and a resident of Zone 4,

Poblacion, Laguindingan, Misamis Oriental, after sworn to in accordance of law, do
hereby depose and say:

1. That I am one of the heirs of the deceased JESSIE C. CAGATAN who died
intestate on January 25, 2016;
2. That on April 26, 2016, the heirs of the deceased executed an Extrajudicial
Settlement of Estate with Special Power of Attorney, acknowledged before
Atty. Roy I. Macua, a Notary Public, with Document No. 247, Page 50, Book
I, Series of 2016 of his Notarial Register;
3. That the heirs authorized me, by virtue of the above-described document, to
withdraw the amount of savings deposited by the deceased with the UCPB-
Savings, Agora Branch, Agora Cagayan de Oro City under Savings Account
No. 810-01-02170-0;
4. That being the surviving heirs, we, the heirs appearing in the above-
described document, are the only persons legally entitled to inherit the
savings of Jessie C. Cagaran Savings Account No. 810-01-02170-0 at UCPB-
Agora Branch.
5. That to the best of my knowledge, no other person is rightfully entitled to the
money under the said savings account of Jessie C. Cagatan except us, being
the surviving heirs of the decedent.
6. That upon receipt of the proceeds of said savings account, I, a duly
authorized representative, undertake to render the Bank free and harmless
against any and all claims by third parties whatsoever.
7. Wherefore, for and in consideration of the foregoing premises, and upon my
actual receipt of the proceeds of Savings Account Number 810-01-02170-0, I
hereby freely and voluntarily RENOUNCE, WAIVE, QUITCLAIM, and FOREVER
RELEASE any and all claims and/or causes of action which I may have or
hereafter have, against the Bank, its officers and agents, over Savings
Account Number1810-01-02170-0, in a manner that is absolute and
irrevocable.
8. That I am executing this affidavit to attest to the truth and veracity of the
foregoing statement and for whatever legal purpose that this may serve.

IN WITNESS WHEREOF, I have hereunto affixed my hand this 9th day of

March, 2018 in Cagayan de Oro City, Philippines.
