Greetings of Peace.

I am Karen Faye T. Tayag, a registered nurse and the current project nurse of Global Project Incorporated with the
proposed SM Center Dagupan Project located in Dagupan City, Pangasinan. I was employed last August 10, 2017.

I want to formally inform your good office about my decision to resign on my current post effective on February
17, 2018 due to some personal and career-growth considerations. In relation with this, may I request from your
good office a certificate of my employment as well as the copies of my paid benefits (SSS, PhilHealth and
PAGIBIG) as they were deducted in my payroll reflected on my pay slips but were not available when I have
verified them in their respective offices.

I greatly enjoyed my whole stay in your company and I appreciate all the shared knowledge and skills that I have
learned. Moreover, I am thankful to all the workforce team members of Global Project Incorporated- Dagupan
Project for the smooth working relationship and friendship that I have experienced. It was a great pleasure to be
part of G.P.I. Dagupan family. In addition, I will properly make a turnover to the next project nurse in the project if
he/she is available until my last work day.
