1. I am a law abiding citizen and a resident of the Republic of the Philippines;

2. This affidavit is executed in behalf of VICTRICIA V. RALLECA-MADARCOS, of legal age and

resident of Brgy Nalasin, Paoay, Ilocos Norte and MARY BETH R. MADARCOS
BALLESTEROS, also of legal age, married and now residing in Ekika Road, Mountain View

3. I am an older sister of VICTRICIA V. RALLECA-MADARCOS ;

4. VICTRICIA V. RALLECA-MADARCOS gave birth to a lovely daughter named MARY BETH R.

MADARCOS last August 25, 1970;

5. I was the one who breastfed MARY BETH since she was born until such time that
VICTRICIA V. RALLECA-MADARCOS was very capable to breastfed her own daughter;

6. VICTRICIA was not capable of breastfeeding her daughter because she was very sick
then;

7. I took care of MARY BETH R. MADARCOS during her early days and months;

8. This affidavit is being made by me for the purpose of assuring that the names BEATRICIA
V. RALLECA-MADARCOS and VICTRICIA V. RALLECA-MADARCOS refer to one and the
same person;

9. That the above named person, BEATRICIA V. RALLECA-MADARCOS/VICTRICIA V.

RALLECA-MADARCOS is the mother of MARY BETH R. MADARCOS

7. I served as the second parent of all the children of Mariano and Victricia Madarcos;

8. This affidavit is being made by me for the purpose of assuring that the names BEATRICIA

asking for my assistance due to the giving birth of his wife

7. I heeded the call and successfully assisted VICTRICIA in her delivery of a very healthy
baby girl which was later on named MARY BETH R. MADARCOS ;

8. This affidavit is being made by me for the purpose of assuring that the names BEATRICIA
V. RALLECA-MADARCOS and VICTRICIA V. RALLECA-MADARCOS refer to one and the
same person;

9. That the above named person, BEATRICIA V. RALLECA-MADARCOS/VICTRICIA V.

RALLECA-MADARCOS was employed as Agricultural Technologist;

5. She served as such from April 1, 1993 to August 7, 2010;

6. Her assignment was at the Municipality of San Nicolas, Province of Ilocos Norte;

7. When she was first employed in this municipality, she used the name BEATRICIA V.
RALLECA-MADARCOS but when she found out that her name in the birth certificate is
VICTRICIA, she caused or requested for the correction of name in the Civil Service
Commission which duly issued an order, CPI No. 2002-750, to correct all the records in the
government pertaining to her name;

8. The discrepancy was then rectified, reflecting now the VICTRICIA R. MADARCOS as the
true and correct name;

9. I am therefore executing this affidavit to attest to the truth of the foregoing, as well as
for all legal purposes this may be worth.

I, VICTRICIA V. RALLECA-MADARCOS, of legal age, married, Filipino citizen and a

resident of Brgy. Nalasin, Paoay, Ilocos Norte, Philippines under oath and states that:

1. I am a law abiding citizen and resident of the Republic of the Philippines;

2. I am a retird Agricultural Technologist of the Municipality of San Nicolas, Province

of Ilocos Norte; ;

3. I served as Agricultural Technologist from April 1, 1993 to August 7, 2010;

4. I have been using the name BEATRICIA since my elementary grades up to when I
was first employed in this municipality of San Nicolas.

5. Sometime in 2002, Bureau of Internal Revenue requested that all employees to

submit their Birth Certificate, it was only then that I found out that my name in
the birth certificate is “VICTRICIA”;

6. Immediately then I caused or requested for the correction of my name in the Civil
Service Commission which duly issued an order, CPI No. 2002-750, to correct all
the records in the government pertaining to my name;

7. The discrepancy was then rectified, reflecting now the :VICTRICIA R.

MADARCOS” as the true and correct name;

8. Since then, I followed my registered name ““VICTRICIA” to avoid complication and

inconveniences and to be consistent with all my other records;

9. Further, I also filed a petition, Correction of Clerical Error in the Marriage

Certificate particularly the wife’s first name from “BEATRICIA” to “VICTRICIA” to
the Local Civil Registry Office of the Municipality of Paoay, Province of Ilocos
Norte last January 20, 2010;

10. A decision dated February 3, 2010 to the petition was issued by the said office
rectifying the error and has become final and executory. Hence, the wife’s first
name in the marriage certificate now reflects from “BEATRICIA” to “VICTRICIA”;

11. Furthermore, pursuant to the Decision rendered by the Municipal Civil Registry
dated October 03, 2012 and affirmed by the CRG under OCRG No. 12-0963263, the
Mother’s First Name was corrected from “BEATRICIA” to “VICTRICIA” in the
Birth Certificate of Mary Beth R. Madarcos;
 12. I am therefore executing this affidavit to attest to the truth of the foregoing, as
well as for all legal purposes this may be worth.
