Identity Documents including Address Verification

From 1 October 2018 Accountants must obtain and verify information from new and existing clients including
full name, date of birth and residential address and if the client is a trust or company that information is
required for persons associated with that entity (such as directors and shareholders, trustees and
beneficiaries).

In order to verify your identity we need you to provide proof of your identity and residential address from only
one of the options below:

All forms of photographic ID listed above and used for ID verification must be the original document and
currently valid.

The residential address verification document must be an original document and cannot be printed from an
online source or in PDF form.

In the event that you are unable to provide any of the above information please contact the office and we
will arrange for alternative procedures.

Document certification
If you are able to bring your original documents to our office in person we will certify copies.
In the event that you are unable to bring original documents to our office in person (e.g. out of town) you
must have copies of the original documents certified face to face by a Third Party Referee selected from
the list below:

 Lawyers  Justice of the Peace  Notary Public

For identity verification the Third Party Referee must sight the original identification and make a statement
to the effect –

“I hereby certify that this is a true copy of the original documentation for

……………………………………......................................
which I have sighted, and it represents a true likeness of the individual.”

For document verification the Third Party Referee must sight the original document and make a statement

The document certification must be carried out in the 3 months prior to the date of presentation of the
documents. The certified documents can be scanned and emailed to Lawson Accounting, kay@lawacc.co.nz
however the original certified documents must be forwarded by mail to Lawson Accounting, PO Box 462,
Timaru 7940.
