Subject: Request for Issuance of Certificate of

Honorably Discharged from the PC, CIS

Greetings Sir!

I am Ex-TSg Victor Oredina Arellano PC with Serial # 618105 former member of the defunct
Philippine Constabulary.

I rendered service from November 1975 to October 1990 with numerous promotion and my last
unit assignment prior discharged from the service was in Criminal Investigation Service (CIS) Region II,
now Criminal Investigation Detection Group (CIDG).

With the transition of the PC-INP to Philippine National Police (PNP), I did not pursue to be
absorbed therein as I was no longer interested to continue my service, hence, was discharged 16 October
1990 from the service, and was living as an ordinary civilian only;

However, when I secured a copy of my service record, as requirement for my application for PVAO
pension, it appears that the same had no mode of discharge whether “honorable”, but am very sure that
my discharged from the service was not dishonorable as there was no derogatory records on file against
me as I have not been charge of any criminal or administrative case, but the omission of the word
“honorable” was merely inadvertence or clerical error.

In view thereof, I respectfully request for the issuance of a certification from your goof Office that
the mode of my discharged from the service was that by virtue of the supervening law creating the new
PNP and the eventual abolishment of the PC-INP.

Your kind approval and consideration with my above request will be highly appreciated.

Respectfully yours,

VICTOR OREDINA ARELLANO
