I am respectfully requesting approval from your good office for the availment of two (2) of
my six (6) credits of Compensatory Day – Off Privilege accrued in consideration of my
travel to Bansud, Oriental Mindoro (with inclusive dates April 29 – May 04, 2018 with RTO
No. 465, and June 30 – July 02, 2018 with RTO No. 841) to conduct site visit and attend
RPMT-CPMT Meeting at the MIMAROPA Youth Center.

Breakdown:
April 29, 2018 Sunday Travel Time CLAIMED
May 01, 2018 Tuesday Labor Day (Holiday) UNCLAIMED
June 30, 2018 Saturday Travel Time UNCLAIMED

I will utilize the CDO privilege acquired last May 01 and June 30, 2018 to off-set the target
date of absence this coming October 18-19, 2018 due to important family matters.
