Management and Patient Knowledge of Osteoporosis

Osteoporosis is a condition that is defined by a loss of bone substance that renders the
entire bone weaker mechanically thus more likely to fracture.

When osteoporosis exists in bones there is a greater chance of a fracture happening under
relative normal conditions, meaning that a less serious trauma is needed to create a hip or
wrist fracture. An adult who trips and fall forward will likely put out their hands to protect
the fall and with normal bone strength it is unlikely that a wrist fracture will occur. That
same adult in later years with weakened bone due to osteoporosis will likely fracture one or
both wrists from a similar fall. Fracture may occur without a single event due to repetitive
force to a bone that is significantly weakened by osteoporosis- this may be referred to as a
stress or fatigue fracture. Fractures occur in bones with normal mechanical strength but
there is greater external force (trauma) needed when compared with a bone weakened by
osteoporosis.

The diagnosis of osteoporosis was established based on the classical values of bone mineral
density (BMD) achieved in the bone densitometry, so that osteoporosis was considered
when the T-score of less than -2.5 SD (the T number of standard deviations that a subject
deviates from the average BMD of a population group of healthy young women).

Some of the risk factors that have been described for osteoporosis, such as age, sex, and
race, are intrinsic and therefore cannot be changed; others, such as diet, life- style,
situations that increase the risk of falls, or the con- sumption of certain drugs, are extrinsic
and can be modi- fied. Moreover, the evaluation of several risk factors may be as important
as the value of bone density screening.

In addition to a public health approach to osteoporosis there are numerous advances in


medical management, nutritional support, exercise therapy, fall prevention and post-
surgery rehabilitation. The attention of the medical community to the consequences of
osteoporosis have helped support the many stakeholders in society who are trying to lessen
the harm that comes from not treating or simply neglecting osteoporosis.
Implant treatment in patients with osteoporosis

Some people said that osteoporosis have an effect to someone who have treatment of
dental implant. Osteoporosis maybe one of contraindication of dental implant because
patient with osteoporosis habe an abnormally structure of bone, especially in cell of bone,
so osseointegration between dental implant and jaw cant united.

This is research about it, doing by Shibili et al. He doing a research with compare patients
normal that do implant dental and patients of dental implant that have osteoporosis. And,
the result is osteoporosis not considered as desease of osseointegration dental implant. The
succesed of osseointegration is caused by health status patient. Althought prevalention of
osteoporosis always increase after menopouse.

So, osteoporosis not inclued contraindication of dental implant. Some of steps must be get,
before begin an implant. That is not different between patient normal that doing implant
with patient osteoporosis that doing implant. But, the oral hygiene must be doinh with
patient before get the treatment. Althought the risk of ONJ onpatient with BP is low, patient
must be given information and give her or his signature on statement letter which contain
it.
