AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This contract is made and entered into by and between:

RAGNAR JOSEPH PARAS, of legal age, Filipino citizen, married and a resident of Brgy.
Capaya, Angeles City, hereinafter referred to as the FIRST PARTY;

- and –

KAREN C . PARAS, of legal age, Filipino citizen, married, and with residence and postal
address at Brgy. Capaya, Angeles City, hereinafter referred to as the SECOND PARTY:

- and –

MARINA R. DELA FUENTE. of legal age, Filipino citizen, married, and with residence and
postal address at Brgy. Sta. Monica, Concepcion, Tarlac, hereinafter referred to as the THIRD PARTY:

WITNESSETH:

Whereas, the FIRST PARTY is the assured and holder of Policy No. BANK-NIL-ANG-0001664
(Personal Accident Policy) from the PNB General Insurer’s Co., Inc, for a sum of TWO MILLION ONE
HUNDRED THIRTY THREE THOUSAND FIVE HUNDRED PESOS (P2,133,500.00);

Whereas, the SECOND PARTY is the beneficiary of the aforementioned insurance;

Whereas, the THIRD PARTY is the designated Representative of the FIRST and SECOND
PARTY.

Whereas, in case of death as provided for in the provisions of the aforementioned Personal
Accident Insurance, which is made part of this Agreement, the herein FIRST PARTY (assured) and
SECOND PARTY (beneficiary) designate the THIRD PARTY, MARINA R. DELA FUENTE, as their legal
representative, with authority and power to negotiate and transact with the FIRST PARTY’s
employer, WITH THE Insurance company and with the concerned government agencies in the FIRST
PARTY’s place of work (Algeria), for the following purposes:

1) To repatriate the body of the Assured, in case of death and his personal
belongings.
2) To claim and receive in behalf of the Assured and/or Beneficiary, heirs,
successors and assigns, all claims from the aforesaid insurance company.
3) To pay from the proceeds of the insurance benefits, the necessary expenses in
the repatriation of the body of the Assured and his belongings.
4) To remit and turn over to the Beneficiary or to his family the excess of the
proceeds over the expenses incurred in accomplishing the purposes for which
this Agreement and Authority was executed and granted.

Whereas, MARINA R. DELA FUENTE, hereby accepts the foregoing responsibilities and
obligations and undertakes to faithfully comply with the same.

IN WITNESS WHEREOF, the parties hereby set their hands this __th day of April, 2009, in
Concepcion, Tarlac.
