Application Requirements for Clinical Electives

San Fernando Medical School


Student Exchange Division

Applicants to the Exchange Program are required to complete the following requirements.

Deadline: March 30th, 2018. 23:59hrs GMT-5 (Lima, Peru)

1. Applicants must be currently registered at least in the 4th year of medical school.

2. Applicants must be in the upper third of their class, the following documents must be
included:

a. A copy of "Promedio Ponderado y Orden de Mérito". (Obtained at Unidad de


Trámite Documentario”, San Fernando Medical School, telephone: 619-7000 ext.
4648).

b. A copy of School Transcripts (“Historial Académico”) downloaded from the website


'SUM' (Sistema Único de Matrícula: http://sum.unmsm.edu.pe/).

3. Applicants must be proficient in English language.

a. Please attach a certificate of completion of an intermediate/ advanced English


course.

b. It is strongly recommended, if available at application date, to provide us a copy


of your last TOEFL score. Keep in mind that two of the host universities will require
it for acceptance.

Selected students to do electives at University of Rochester will be asked to present


TOEFL registration up to 7 days after the results are communicated, and the exam
must be taken within one month.

4. Resume / Curriculum vitae (suggested length: 1 page. Must be written in English.)

a. Include year of entrance, current class, work and volunteer experience. Also
include publications in indexed/non-indexed journals, books/poster presentations (attach
any supporting documentation), awards, interests, and skills.

5. Personal Statement (300-500 words. Must be written in English.)

a. We encourage you to convey personal thoughts regarding your motivations and


future professional goals. We want to know who you are, what you have to give, what makes
you desire to engage in an elective in the U.S, and what would you wish to do for a career
in medicine. Must be a self-written, original document; any trace of plagiarism may disqualify
you from the entire process.

6. Selected applicants will be then required to undergo a Personal Interview.
