utilization of emergency room visits and inpatient hospital stays; improved decrease in heart failure re-
admissions; reduced re-admissions for chronic obstructive pulmonary disease patients; improved patient
engagement and care participation for self-management goals; improved patient centered medical home
(PCMH) patient and provider engagement.

Binghamton, New York – Provided an overview presentation of new position and responsibilities within the
organization for staff not familiar with role.
Childhood Obesity: Assessment and Treatment in a Physician Network System, 2012 Binghamton Biomedical
Research Conference, April 27-28, 2012, Binghamton, New York – As part of a grant funded project with the
Strategic Alliance for Health, provided a podium and poster presentation to present work done to address
pediatric obesity in a pediatric office, of which was part of a larger healthcare system. System changes and
information on how to disseminate work done which produced a reduction in the number of children classified
as obese during our measurement period was provided, as well as plans to disseminate work throughout an
organization’s physician network.
NYSIIS Presentation for Staff Education, Our Lady of Lourdes, May 2009, Binghamton, New York – Provided
education to clinical staff and providers regarding the change to a web-based immunization registry system for
New York State; the public health law requiring providers to report to the immunization registry; as well as how
to obtain access and utilize the online registry for data entry and reporting.
