2.1 Definisi
sepenuhnya jelas
(http://www.liv.ac.uk/HumanAnatomy/phd/research/perthes/perthpd.html).
2.3 Faktor Risiko

socio-economic deprivation
hyper-coagulable states
Weak
urban population
The femoral head relies on lateral epiphyseal vessels for its blood
supply between 4 and 7 years of age. These vessels run in the
retinacula and are susceptible to stretching and pressure in the event
of an effusion. [27] [28] [29]

2.5 Patofisiologi
Children with Perthes' Disease have been found to exhibit growth delay
affecting both their skeletal maturity as well as their anthropometric status.
Research in Liverpool has also shown that the feet are small in the sufferer when
compared to their peer group. Other growth paramenters ehibit a tendency for
smallness when compared to normal unaffectd children

attain puberty and a normal final height by the mid teens.

that innervate the hip joint; it may radiate to the suprapatellar
portion of the knee through the femoral nerve. This can lead to
mistaken evaluation of the knee and a delay in diagnosis.

diagnosis.

MRI of hips
Should be considered if radiographs appear
normal. It is also a useful adjunct in the early
stages of diagnosis, and has been found to be a
sensitive modality in diagnosing Perthes' femoral head collapse
disease. [54] If performed 6 months after disease and fragmentation
onset, an MRI can accurately demonstrate the
degree of epiphyseal involvement. [55] After
reossification, MRI of the hips may also be
useful in assessing the extent of the damage in
one or both hips.View imageView image

2.7 Prognosis
The strongest predictor of outcome was femoral head involvement of more or less
than 50% (odds ratio (OR) = 7.76, 95% confidence interval (CI) 2.82 to 21.37),
followed by age at diagnosis (OR = 0.98, 95% CI 0.92 to 0.99) and the lateral
pillar classification (OR = 0.62, 95% CI 0.40 to 0.98). In children over six years at
diagnosis with more than 50% of femoral head necrosis, proximal femoral varus
osteotomy gave a significantly better outcome than orthosis (p = 0.001) or
physiotherapy (p = 0.001). There was no significant difference between the
physiotherapy and orthosis groups (p = 0.36), and we found no difference in
outcome after any of the treatments in children under six years (p = 0.73).
The
abduction orthosis should be abandoned in Perthes’ disease.

Pasien
biasanya memiliki fenotip karakteristik perawakan pendek, usia tulang tertunda,
dan hiperaktivitas. Perthes biasanya bersifat unilateral, kecuali pada 10-20 persen
kasus yag ditemui.
