according to Age, Sex, Academic Track, Grade Level, and Family Income of the
respondents?
6. Is there a significant relationship between the Academic Performance and
Mental Health status among Senior High School students in SAC?

Procedures:

As a participant, you will be provided with the questions relevant to the study. The
instructions as regards the various questions in each section are stated properly or will
be explained clearly. There are two parts in the Questionnaire: Section A –
Demographic Information, Section B – Academic Performance, and Section C – Mental
Health. You are expected to answer the questions or items honestly, responses that are
most applicable to you. There is no right or wrong answer. The language used is simple.
All these items will take you from 10 to 15 minutes.

Volunteerism:

You may decline to answer any or all questions and you may terminate your
involvement at any time if you choose to.

Benefits:

As a participant, you will be part of the success of this whole study. The
information you will provide is of great help as part of the body of knowledge that
others or future researchers may benefit. Although, there will be no direct benefit to you
for your participation in this study, yet, the researchers hope that the obtained findings
