To Whom It May Concern:

This letter accompanies my formal claim to cover the cost repairing my car Ford Everest
Ambiente AT 4x2, after a car on convoy, Chevrolet Sail bump into my car damaging the right
head light, fog lamp, front bumper, bumper bracket, fender lines and mud guard, which I
acquired last January 05, 2018 at Ford Manila. The Ford Everest was damage tremendously as
it was side swiped at the left rear end of Chevrolet sail. Drivers are my senior citizen uncle for
the Ford Everest and my younger brother Floyd Jaron for the Chevrolet. Both are approaching
going out in the vicinity of our subdivision. My uncle suddenly stop in the middle while driving,
my brother who is at the back driving the Ford Everest failed to stop and crashed into the back
of Chevrolet Sail.

I called the police and reported it with police blotter .Then, the next day we immediately
transported this to Ford Gensan upon talking with the service provider of Ford Gensan. They
could not give us any specific date on when would possibly the car will be repair since the car
was new and acquired within this year. They had given us 4 to 5 months or more until we can
get the car back. I am 1 month pregnant during that time and my doctor advise me to take really
good care because my pregnancy is at high risk. She advised me to take duphaston
(pampakapit) because I was bleeding on my first trimester of pregnancy. We badly needed the
car. Since our domicile is located in remote area of locality though it is within the city proper. It
would be exhausting for me to commute everyday going to the office (DENR XII) together with
my daughter who is a student (kinder II). My husband is not always around with us because he
is an OFW. We badly needed the service vehicle.

The last recourse we can do is to repair it outside FORD dealer. The owner of the shop
is a mechanical engineer and one of the most well-known shops in the City of Koronadal.

Our goal is to settle this matter quickly and efficiently. Issuing LOA is the best remedy
we can do to make this transaction faster. We badly need our service vehicle for our day to day
transactions. We look forward for your immediate action on the matter.
