Facilitate offering of quality advice on the Program and the best innovative pathways based on the identification of sites
needs for continuous quality improvement

Reports to the Hospital Administrator/CEO
Purpose:
To ensure highest standards of HIV testing services, Care and treatment, viral suppression, adherence, differentiated model of care,
infection prevention and Biosafety standards are maintained and constantly being reviewed through the continuous quality meetings.
Support and facilitate the HIV testing both at the OPD (including special Clinics) and IPD
Support and strengthen the monthly data reviews of the linkage at the Clinic
Enrol and enhance adherence counselling to HIV positive clients / patients
Clinical assessment and treatment of HIV positive patients
Run HIV related services – TB Clinics, PMTCT Clinics, STD Clinics and any related services
Ensure various registers, patients files and documentation tools are correctly filled in and updated for improved quality of care
Strengthen and monitor the laboratory patient monitoring systems e.g. Viral loads, gene expert.
Work closely with the community team to prioritize community home visits, treatment preparations and locate the lost to follow
up
Establish and lead a multidisciplinary clinical teams to review the treatment failures on a weekly basis.
Strengthen the continuous quality improvement initiatives by leading the Clinical team in identification, review and
implementation of the various quality improvement frameworks.
Participate in facility data review exercises in the bid to identify and improve the care and treatment targets set by the donor
Participate and support the facility reporting that is done on a monthly, quarterly, semi-annually and annually (especially
mortality, crag).
Any other duties that are in line with the program deliverables, allocated by the administration
Additional attributes:
- At least 1 year experience working in a donor funded ART program.
- Experience on the use of IQCare Clinical databases and or any EMR system in patient data storage and retrieval system.
Duties for a Lead Clinician
Goal of the position:
General oversight of the Clinical arm of the Clinic, to ensure high quality and achievement of HTS, care and treatments, viral suppression
targets, infection prevention, rational use of medications by continuously reviewing quality of services through CQI meetings.
Duties and Responsibilities;
Establish and chair continuous quality improvement meetings, infection prevention, drug use, MDT, data review meetings on a
monthly basis. Ensure the minutes are well kept and shared.
Ensure high quality of the Clinic performance by ensuring the HTS, Care and treatment and viral suppression targets are achieved
in accordance with the UNAIDS 90-90-90 initiative.
Oversight to the implementation of differentiated model of care, Nursing services, drugs and commodity management, and the
biosafety measures at the Clinic.
The lead clinician or designate will lead ward case reviews as appropriate, set and convene CMEs, and lead in ward cases
discussions and the lead clinician will be the contact point person when consulting with the TA team on difficult cases
Structured and monitored mentorship of the facility and satellite staff to ensure implementation of the National HIV treatment
guidelines provided for by NASCOP.
Ensure all documentations in the various patient monitoring tools are correctly filled in
Ensure laboratory patients monitoring systems are well done and documented
Ensure timely and accurate reporting of all the Clinical indicators
Ensure achievements of various targets provided by the Program
He or she will Represent the site in Clinical Forums in County of Sub county in the absence of the coordinator or as assigned
by the coordinator
PROJECT ACCOUNTANT.
1. JOB OBJECTIVES:
To provide financial information to management by analyzing accounting data and preparing reports.
1. Prepares analyze and compile all account entries and information to recommend required financial actions.
2. Initiates corrections as advised by the Senior Accountant when invalid accounts structures are determined
3. Summarizes current Program financial status by collecting information for reporting to the Senior Accountant; reports
include cashbooks, budget status , advances and payables schedules and bank reconciliations by the 5th of each month.
4. Prepares financial, budgetary, and/or encumbrance journals as required to correct coding problems to ensure proper financial
reporting.
5. Maintains accounting controls by preparing and recommending policies and procedures.
6. Performing bank reconciliations and maintaining petty cash records and cash certificates.
7. Prepares payments by verifying documentation, and requesting disbursements.
8. Answers accounting procedure questions by researching and interpreting accounting policy and regulations in line with
donor requirements.
9. Performs procurement of goods and services as per USG Rules and regulations.
10. Prepares special financial reports by collecting, analyzing, and summarizing account information and trends.
11. Responsible for building, maintaining and coordination positive relationship between finance and other departments to
ensure achievement of organization’s goals and objectives.
12. Liaise and provide relevant information to Internal Auditor and External Auditors.
11. Preparing budgets, perform monitoring and tracking expenses to the budget to enable management to maintain appropriate
burn rates.
KEY RESPONSIBILITIES
Purpose:
To maintain, accurately capture, record, stored and retrieve data to ensure prompt reporting of information and statistics to relevant
authorities
Duties and Responsibilities:
1. Collect and tally required data related to a project providing antiretroviral therapy through daily data update on the IQCare
System
2. Data Validation on MOH Register and IQCare
3. Preparing of monthly and quarterly reports which is sent to MOH and KARP Regional team

5. Timely reporting of defaulters to the social Worker on a weekly basis
6. Involved as a lead member in the QA/QI committee (Secretary)
7. Make orders for medical forms used in the project i.e. Social Demographic forms, Initial Evaluation Forms, follow up forms
,home visit forms ,pharmacy forms and lab forms
8. In collaboration with clinicians ,nurses ,pharmacist, assist in tracing of clients files through the use of IQCare system
9. Attend and actively represent the project and hospital in seminars /meetings involving partners and stakeholders or the
government.

LEAD NURSE
Professional qualification: a minimum of KRCHN/KRN/M/CH or equivalent; recognized by regulatory body (NCK); Has current
valid license

Responsible for: Lead clinician
Goal: Offer an oversight comprehensive and quality Nursing and HIV testing services
Objective: Offer effective leadership to the Clinic Nursing, adherence and HTS
Nursing responsibilities for the lead nurse involve but not limited to the following:

1. Organize Clinic & form a Clinic Flow
Facilitate one directional clinic flow in respect to differentiated care model(DCM)
Address all bottle necks to facilitate daily patients’ flow
2. Establish & Supervise Triage
Participate and audit triage activities: vital signs, pill count, fast tracking
Establish triage equipment & their efficiency
3. Formulate schedules for the clinic, nursing & subordinate staff
Organize for a functional duty rota to include leave, day offs for all nursing staff and the clinic adherence team
Organize and harmonize clinic demands with the existing staff
4. Participate in program coordinating team
Represent adherence /nursing component in meetings
Advice on nursing /clinic adherence issues
Support decision making process in the clinic
Adherence multidisciplinary team
5. Maintain Supplies in the Clinic e.g. Stationaries, Gloves etc.
Organize for smaller facilities supply if supported
Order for clinical supplies on time through the existing institution purchase channel.
6. Plan or coordinate clinic CME for the nursing staff
CME for the nursing/ and adherence team-Continuous nursing education
Involve the other staff in facilitation
7. Offer oversight to all HTS related activities in the facility
Involve HTS lead in establishing whether screening, testing and linkage of the identified is taking place
Trouble shoot to establish if there are any impediments and sort them out
Organize and implement a CQI challenge for the HTS providers
8. Formulate Standard Operating Procedures and annually review them to suit national guidelines
To standardize the procedures done in the clinic
To synchronize CCC standards with acceptable professional standards
9. Periodically analyze the patients’ Waiting Time in the Clinic and conduct client satisfaction surveys
Use questionnaires, observation methods to assess the patients’ waiting time
Work on the service flow by involving the whole team
Improve on some bottle necks in the patient flow
10. Conduct Nursing and Adherence Mentorship to all supported smaller facilities
Mentor staff working in smaller facilities on triage, adherence & task shifting
Assist in smaller sites organization in matters of:
o Staffing
o Supplies
o Systems strengthening
o Quality nursing services
11. Supervision & active participation in Quality Assessment
Assist and be actively involved in setting up or supporting a quality assurance team
Assist and implement small test of change (STOC) done at the site
Be part of the working group (system) to encourage team work
12. Submit monthly reports to the Coordinator through the lead clinician on:
Clinic general progress
Status of equipment
Human resource
Achieved Targets status
Status of capacity transfer among staff
13. Reporting STOC to the Director of Nursing on matters concerning Nurses; all protocols observed
14. Be key in all matters around Infection Control at the CCC
Ensure there is circulation in waiting bay, clinical rooms & promote smooth one directional service flow
Promote hand hygiene through Hand washing
Enhance active representation & participation of Nurses in Infection control forum
15. Other responsibilities as delegated to by the Supervisor

Job Purpose: To offer effective Nursing services to clients on day to day basis anchored on Triage and task shifting models
1. Assist team in the daily provision of medical care including ART. Tasks will include triage, counseling, nutrition
assessment, scheduling and assurance of appropriate laboratory evaluation, data collection and assisting other clinic staff
with procedures.
2. Together with other team members plan for smaller site visits including making sure equipment required are available.
3. Ensure supplies and equipment for use in the clinic and satellite are available by liaising with the lead Nurse-: e.g.
thermometers, sphygmomanometers, stethoscopes, weighing scales, height scales, gloves files, stationery
4. Be responsible for the day’s running of the triage services at the CCC
5. Use daily appointment list or diaries to help track missed appointments
6. Submit at the end of day all the names of clients missing appointment to the longitudinal nurse
7. Monitor clinic activities and give feedback to the lead Nurse
8. Ensure clinic activities are carried out effectively and efficiently by all staff
9. Monitor care of the patients/clients in consultation with the other staff to ensure quality
10. Organizes files reviews for follow-up on specific activities carried out
11. Support other nurses and CHVs in the clinic in execution of daily duties.
12. Conduct clinic-based learning sessions (health messages) designed to educate patients and CME/CPD for staff
13. Ensure booking and follow-up of priority patients is done by working/liaising with longitudinal nurse and CHVs.

HTS COUNSELOR
Job summary: Reporting to the Lead HTS, the counselor will be responsible for Screening, Testing and Linking to treatment the
identified Patient or client both at facility level and targeted community level.
Goal: Facilitate optimization of HTS intra facility and targeted Community testing
Desirable: offer HTS to all clients and patients attending a health facility for whatever reason as part of routine care and extend this to
the targeted community testing and ensure linkage of all identified.
Work hours: 8.00 am-5. 00p.m
Job Experience
At least 1 year experience in providing HIV counseling and testing in a government or mission health facility
Excellent and proven written and verbal skills in English

Qualifications:
Overall job function: Oversee facility implementation of TB prevention, diagnosis and management activities
Key activities and responsibilities:
1. Facilitate, planning and implementation of HIV and TB care and treatment services and activities at facility level
2. Provide on-going support supervision of HIV/TB care and treatment programs at the facility including IPT and presumptive TB
case identification
3. Preparation and submission of site work plans, reports and other technical papers
4. Liaising and consulting with the sub-county TB coordinator to ensure timely problem solving and provision of efficient and
effective TB services
5. Ensure timely and monthly discussion of TB associated mortalities
6. Coordinate universal INH prophylaxis therapy implementation, outcome evaluation and reporting
7. Ensure proper documentation in the IPT, TB and presumptive TB registers
8. Make timely commodity reports and ordering
9. Collaborate with the CQI lead and facility IPC lead to ensure TB prevention and control measures are observed
10. Coordinate active TB case finding activities at facility and community levels, documentation and reporting of presumptive TB
cases
11. Co-ordinate mentorship of facility staff whenever called upon
12. Provide leadership for routine TB/HIV CQI audits focusing on quality of CQI, IPT, TB/HIV management(ART initiation),
nutritional monitoring, gene xpert uptake, documentation and report validation
13. Provide continuous support for DOTs for eligible patients through household members and CHVs
14. Conduct DOT for MDR patients
15. Provide leadership for MDR TB surveillance at facility and community levels
16. Any other reasonable tasks as assigned by the line manager
Qualifications:
PMTCT nurse will be based at the facility in MNCH department and will work in collaboration with Mentor mothers in the
department and Community Health Volunteers (CHVs) who are based in the community.
PMTCT nurses and Mentor mothers will work together to ensure provision of PMTCT services to clients in MNCH which include
HTS, inter and intra facility linkage and facility-community linkage to support clients, increase demand for services and educate on
PMTCT and related health topics within the facilities.
Requirements
Mentor mothers will be based at the facility in MNCH department and will work in collaboration with Community Health Volunteers
(CHVs) who are based in the community.
Mentor mothers and CHVs collaborate with and complement each other by providing linkage between facilities and communities to
support clients, increase demand for services and educate on PMTCT and related health topics within the facilities and in the
community.
17. Provide one-on-one peer education and psychosocial support to PMTCT clients and couples.
18. Co-facilitate PMTCT support groups within the facilities.
19. Facilitate group pre-test counselling education for pregnant and breastfeeding women in MNCH
20. Co-facilitate group health talks in facility waiting areas on key messages in PMTCT at the MNCH
21. Refer PMTCT and MNCH clients for other services within the health facility and community based services
22. Encourage partner involvement in MNCH services, education and psychosocial support (Encourage male partners in MNCH to
be tested, offer Partner Notification Services e.g. invitation cards to MNCH clients)
23. Trace and track priority PMTCT clients and MNCH clients who have dropped out of care at key points along the cascade and
follow up through telephone calls and SMS. Those not successfully reached by phone tracing to be linked to CHVs for home
tracing
24. Assist PMTCT clients with disclosure to their families and spouses.
Requirements

3. Recent PMTCT experience (6 months to 2 years) and the baby’s outcome is HIV NEGATIVE
4. Has disclosed HIV status to at least 1 person within her household and is willing to disclose her status to other clients in
MNCH
5. Lives within the local community
6. Virologically suppressed
7. Family contacts have been tested and HIV status is known (Partner, other children)

DRIVER/MESSENGER

SOCIAL WORKER
Purpose
In line with GIPA (Greater Involvement of People living with HIV/AIDS) protocol, Client mentors will educate, counsel and
supervise other clients on treatment literacy, adherence and preventions agenda.
Duties/roles
1. Client mentors will provide psychosocial support to HIV positive clients as first line counselors and treatment literacy
providers.
2. Work collaboratively with primary care and case managers to identify newly diagnosed clients, offer necessary support,
mentorship and follow up.
3. Provide individual and group educational skills building opportunities to foster adherence to medications, identify strategies
to improve adherence to health routines, communication with providers and additional issues to increase engagement in care
and adherence to treatment.
4. In collaboration with Longitudinal care team, will carry out community mobilization for HIV services utilization.
5. All the information obtained by the Client mentor in the course of his or her work must be kept confidential and breach of
this may result into termination. Shared confidentiality only allowed with the facility’s Technical team.
6. Compiles and submits service reports to longitudinal care lead person in the facility.
7. Provide community PwP/PHDP messages and set prevention goals with clients at every visit and at support group levels.
8. Uses personal experience disclosure or testimony in client or group education and motivation.
9. Manage the filing area to ensure neat and orderly filing according to SOPs.
10. Support orderly file retrieval and addition of new required forms
