Calbayog, And in order to effectively hit the objective set by the management . The
undersigned strongly recommends VSM Chito Avila presently assigned as RDF Central
Leyte PMPMI .

Subject: : Violation Of Code of Discipline Article VI section IV .2 b-Offense

Involving Moral Turpitude/Fraud/Theft particularly
Unauthorized delegation of collection function to anyone else.

This is in reference to the findings from the finance office(DDI Tacloban)

with regards to the withdrawal of stocks( ACE FG -45 ream) manifested by a Delivery
Receipt # 182, but the actual delivery was only 28 reams with the equivalent amount of
Seven Thousand Seven Hundred Fifty Six Pesos. Relying on the system & based on the
accounting records, there is a Credit Memo in the amount of Ph 4, 709.00 ( Four Thousand
Seven Hundred Nine Pesos in order to write off & cover up the shortage.

Please explain the following grounds to wit:

Pertinent to the above findings you are made to explain in writing within 72
hours upon receipt hereof, why you should not be charge, otherwise other diciplinary action will
be filed against you in violation of Code of Ethics.

In compliance with the Travel Order No. 2009-00125, the undersigned went to Brgy.
Uguisan and San Vicente, MacArthur, Leyte dated August 12-13, 2009, to assist the
Cashier in the disbursement of UDP Contractor’s 15% Mobilization fee on ANR,
Reforestation and Agroforestry Projects. On August 14, 2009, we went to the following
Brgys such as : Mag-aso, Panzud, Bungtod & Bukawon which are located in Lapaz,
Leyte, for the same purpose as stated above.
