EMERGENCY RESPONSE MANAGEMENT PROCEDURES

3.1 Foreword

The Emergency Response Procedure document is specifically developed to establish an


effective management of emergencies that may occur at the work-site in the event that the
control system in place had failed to contain the hazards and risk involved in the course of
operations activities.

The procedure listed in the document should cover all aspects of the JV BINA TAR Project
where the vicinity will also utilize the client compound/Area

The document is designed to achieve the following objectives:


 Systematic approach in management of crisis
 Effective response of emergency
 Coordination of external support
 Effectiveness of the decision-making

The procedures outlined in document meets the legal requirement in the provision of safe
work place and satisfy the Project Emergency Procedure and bridging with the
Onsemiconductir ERP in providing adequate procedure and provision in the event of an
emergency. Refer to Appendix 1.

It must be stressed that the procedure do not relieve exonerate all personnel of their
responsibilities towards their own health safety besides having the moral obligation to ensure
the safety of his colleagues and safeguarding the natural environment.

3.2 STANDARD DEFINITIONS

Company : JV Bina

Client : On semiconductor

Emergency : Unexpected condition resulting in serious injury or fatality,


fire, explosion, spillage of oil or chemical, toxic gas release,
structural damage, total evacuation, collision at sea, helicopter
crash etc. which require prompt action to contain the
emergency. The emergency is classified as follows:

Major Emergency
 The emergency that required external assistance after the
inability of the ‘Site Emergency Response Team’ to control the
situation had not been successful.

Minor Emergency
 The emergency that could be dealt with successfully by the
‘emergency coordinator) with the equipment and facilities
available at the work-site, without any external assistance.
The emergency however may escalate into major emergency
should the ability to contain is lost.

Damage Control Manager : The Appointed Person (Chief Operating Officer, in his absence the
General Manager) who is responsible to coordinate the assistance
and guidance in the event of an emergency at work-site. He is to
be assisted by the ‘core team’ made up of Project Operations, ,
Human Resource and HSE.

Core Team Members : The appointed persons from Project Operations, , Human
Resource and HSE section who are responsible to assist the
Damage Control Manager for the coordination of assistance and
guidance in the event of an emergency at work-site.

Emergency Manager : The highest-ranking person at the location shall be the


‘Emergency Manager’ and shall coordinate all the management of
emergency at site. He is responsible to communicate the required
information to the Company and he shall also inform the Client’s
Representative of the same.

Site Emergency Response Team: The group of personnel at the worksite who are responsible to the
Emergency Manager for the containment of the emergency at the
same.

3.3 PRINCIPLE OF EMERGENCY MANAGEMENT

3.3.1 Scope

JV core business is in the construction. All activities are undertaken in accordance with our
policy for safe working environment at all work-site and effective management of all hazards.

Should the controls implemented failed to contain the hazards and risk, the contingency
planning and emergency response system is in place to manage such failure(s).

3.3.2 Emergency Management Policy

It is our policy that all activities are carried out in accordance with safe work procures, in line
with our legal requirements and moral obligation to safeguard the health and safety of
personnel and the protection of the assets and the environment.

It is intended that the ‘Site Emergency Response Team’ shall place the following priorities in
the event of such emergency. They are as follows:

 Priority 1 : The safety of personnel


 Priority 2 : Protection of the environment
 Priority 3 : Protection of the assets
 Priority 4 : Reputation of the company

3.3.3 Emergency Evacuation Philosophy

The Company’s Emergency Evacuation Philosophy is to ensure all personnel can evacuate the
area of the work safely when an emergency situation occur

In the event of emergency, which occurred during the period,

Stage 1

 The Emergency Response Team/emergency coordinator (ERT) is activated to


contain the emergency. All essential personnel are to gather at the assigned ‘temporary
assembly point)’ for head counting and to provide assistance as required.

 Support vessels and tugs to be on standby to provide immediate assistance (if
required by the Emergency Manager)

3.3.4 Overview

The Emergency Manager shall be responsible and accountable for the management of
personnel on Construction site. He is responsible to ensure that all personnel involved are
familiar with the emergency procedure when such the need arises.

He shall ensure that materials, equipment, contract services, specialist support and other
relevant provision are available and in place to cater for the recognized emergency.

The Damage Control Manager Project Manager and Project engineer will manage the
emergency at the site. In addition, he would liaise with the Client’s Emergency Control
Center (and the Third Party) for the support/assistance. He would also liaise with the
relevant authorities, if required.

4.0 ORGANISATION AND GENERAL RESPONSIBILITY

5.1 Project site

5.1.1 Damage Control Manager

The Project Manager, shall assume the duties and responsibilities of the Damage
Control Manager (DCM) at the Head Office in Sunway and working together with the
Onsemi ERT representative.

He is to be assisted by the following personnel who form the ‘Core Team’ for the
support of the emergency management in providing the necessary support for the
management of the emergency that occurred at the work-site, if required. The
personnel are:

1. Project Manager
2. Operations Manager for operations coordination and liaison
3. Mariner & Logistics Manager for marine/logistics coordination and
liaison
4. HSE Manager for safety advisory and the event recording and general
liaison.
5. Engineering Manager for engineering aspect of the activities.

5.2 Worksite

5.2.2 Site Emergency Response Team (SERT)

The safety supervisor on duty shall be in-charge of the Site Emergency Response. He
will conduct the head count at the worksite. He will provide the first line response to
any emergency that occurred at the work-site.

During the emergency, the personnel with emergency duties shall be manning- up
their assigned facilities or supporting equipment in the effort to contain the emergency
within the shortest possible period. They shall report the status of the
facility/equipment and will execute the directives from the Emergency Manager as
required.

5.0 MANUAL & PROCEDURES

As stated, the Emergency Response Plan (ERP) document meets the legal requirement and
moral obligation in providing adequate contingency management planning to manage the
recognized emergencies that may occur in the course of the operations at work-site.

The scope of the emergency management is consistent with the contractual requirements of
our client and in line with their system in emergency management.

6.0 NOTIFICATION OF EMERGENCY

7.1 Declaration of Emergency (work-site)

In the event an emergency occurred at the work-site, the person who witnessed the occurrence
should raise the alarm and immediately notify his Supervisor. The Supervisor then shall
notify the Construction Superintendent of the emergency and should attempt to localize the
same and preventing it from spreading.

The Project Engineer and safety officer and client rep , shall evaluate the situation and declare
the emergency (if necessary) via the alarm by sounding the excavator honking continuously
and mobilize the personnel to contain the emergency.

All personnel, upon bearing the alarm, shall immediately:

(a) Cease his work or activity


(b) Make safe the work-place
(c) Proceed to his respective station
(d) Report his presence to the supervisor-in-charge/emergency coordinator

After head-count of personnel, the Emergency Manager will then execute the necessary steps
and procedures to contain the emergency within the shortest possible period.

During the period, he should have initiated the following actions:

(a) Notify the nearby facilities and vessels of the emergency


(b) Move the barge away from the facility (if moored there) and
(c) The ability of the Barge to manage the situation

Should he (Emergency coordinator ) feels that the situation would be getting out of control, he
would then activate the ‘emergency call-out procedure’ for external assistance, detailing the
current situation of the emergency and the scope of assistance required.

In such an event the external assistance is provided, the responsibility and accountability for
the emergency management still lies with the Emergency Manager who shall coordinate the
internal and external efforts to contain the emergency.

7.2 Message Authorization

The Emergency Manager shall authorize all messages that are communicated to and from the
worksite. This is to avoid miscommunication or misunderstanding arising out of the
messages that had been sent or received.

He shall ensure that the Client Representative is informed of the incident and all events are
duly recorded to aid accident investigation and reporting requirement.

7.3 Message Contents

The contents of emergency notification message shall as a minimum include the followings:
a. Name/person authorizing the message
b. Name of barge/vessel of facility
c. Date, time and location of the barge, vessel or facility
d. The nature of the emergency and its latest status
e. Number, name and condition the emergency
f. Actions taken to contain the emergency
g. Ability sustain the actions – manpower, systems and equipment
h. Scope of external assistance (if require)
i. The prevailing weather condition

Note:
o It is recognized that clear and concise communication plays the most critical role in
managing the emergency by ensuring clear understanding by all parties involved, of
the actual situation. The use of correct words, sentences and terminology is critical in
communicating the information to the relevant parties.

o Whenever possible, all the messages shall be transmitted either by E-mail, fax or telex
from
and to the barge (work site). He use of radio for communicating should be minimized
to avoid garbled or misunderstood information.

8 EMERGENCY CALL-OUT PROCEDURE

8.1 Action Plan

Upon the activation of the Emergency Call-out Procedure by the Emergency Manager, with
the agreement from the On semiconductor representative

(a) Mobile cellular phones/company own’s

Project Manager and Operations Manager’s laptop computer is designated to provide the E-
mail access/facility.

The DCM shall assign the team members of their responsibilities in regards to supporting
actions and collection with other parties e.g. Contractors etc. and On Semi-conductor
Emergency Control Center (ECC) in the management of the emergency support.
