APPLICATION REQUESTING FOR A CHILD BAPTISM

We believe that our child should receive the Sacrament of Christian Baptism, hence we
hereby kindly make this request. And in order that the baptism of our child may be truly a
sacrament, we humbly submit to the call of Christ Jesus to “Let the children come to me and do
not hinder them, for to them belongs the Kingdom of Heaven;” and we make the following
promises as we bring our child to Christ through this holy sacrament.

 We will attend our Church’s Worship Services, Prayer assemblies, and Bible Studies
regularly so that we will learn more about the Evangelical Faith, in which we were
taught and through which our child will be taught, that we may be better prepared
to teach our child the values of Discipleship through Church attendance.
 We will endeavor to study Biblical principles of our Christian faith until they become
clear to us, so that as our child grows we may be better prepared to answer his/her
theologically-related questions, and may properly nurture him/her morally and
spiritually.
 We will have a time to pray with our child, to counsel and guide, to teach and correct
him/her as our child goes to bed, or during meal time, or at some designated time for
this purpose; and allot a time to read to him/her the Holy Scriptures that he/she may
grow in the knowledge and love of the Lord Jesus Christ.
 We will encourage and bring our child to Sunday School Classes,
Communicant/Confirmation Classes, Church Youth Fellowships, Discipleship Classes,
and the like, as our child come of age, in order for him/her to be firmly rooted in
his/her Christian life.
