I am satisfied that the appointee has received adequate training and /or is experienced in correct operation 
safe handling of mobile plant owned or hired by the company. He is hereby authorized to drive /operate any of 
mobile plant show above. (Note the person signing as the “authorized company representative” in this section 
be  satisfied  that  driver/operator/electrician  has  received  adequate  training  or  is  experienced.  This 
appointment is not valid unless they require information has been entered including signatures of appropriate 
personnel.
