AFFIDAVIT
I, JAYCEE C. CRISTALES, of legal age, single, Filipino, with residence at block
24 lot 8, Carmela Valley Homes, Talisay City, Negros Occidental, after having been duly
sworn to in accordance with law, do hereby depose and state the following:

1. That I am a licensed Physical Therapist here in the Philippines and in the

State of Illinois, USA (a copy of my PRC License and Certification from the
State of Illinois is herein attached as Annex “A” and “B’);

2. That sometime in January 15, 2018, I visited the United States of America
(USA) on the strength of a Tourist Visa for a vacation;

3. That while in the US, I took the National Physical Therapy Examination which
I was able to pass;

4. That a month after the said examination, I already sought employment in the
US;

5. That RehabCare Group East, Inc., with principal address at 680 South Fourth
Avenue, Louisville, Kentucy 40202, had a job opening for Physical Therapists
and sponsors H1B Visa for its employees;

6. That I submitted my application as a Physical Therapist with RehabCare

Group East, Inc. and was able to secure a phone interview with them;

7. I successfully passed the interview and was offered the job in

Bement/Decatur/ Sullivan Rehab, State of Illinois, as evidenced by the
Agreement Letter send to me by RehabCare Group East, Inc. heretofore
attached as Annex “C”;

8. That I am executing this affidavit to attest to the truth of the foregoing and
for whatever legal purposes that this statement may serve.
