Sir:

We are pleased to inform you, that your school is identified as one of the Cooperating Schools
in Field Study (FS) courses.

CHED Memorandum Order No. 30 (CMO 30) provides that, “Field Study courses are intended to
provide students with practical learning experiences in which they observe, verify, and reflect on,
in actual school settings. The focus of the above-cited Field Studies is as follows:

 Field Study 1 (FS 1) The Learner's Dev. & Environment

Some of your teachers will be chosen as Resource Teachers, to be observed by FS student

along the area of focus of the FS. The student will not disrupt the regular teaching-learning
activities because his task is merely to observe and document his observations to come up with
the required outputs to be signed by the Resource Teacher/s during their spare moments.

Furthermore, the student agrees not to disclose any matters concerning the cooperating
school(s). Any information gathered during the class observation will be handled with utmost
confidentiality and for the sole purpose of complying with the requirements of the above-
mentioned Field Study course.

In this regard, the undersigned, Diploma in Teaching Secondary Education (DTSEd) student with
a degree Bachelor of Science in Computer Science, through the NSU College of Education
requests your permission to allow him to observe some of your classes within June to October
2017. Further consent is hereby solicited to let him take pictures or videos of things which could
be best captured through electronic gadgets to come up with more reliable, accurate and
authentic documents.

For your kind consideration and anticipated favorable action to this request, we convey our
